负载刺五加源细胞外囊泡的水溶性微针用于糖尿病创面愈合

糖尿病足溃疡是糖尿病最常见的延伸疾病之一。由于糖尿病溃疡伤口处存在的持续炎症和低氧环境,会导致伤口处存在炎症因子高表达,微血管生成受阻及细菌感染等情况。传统方法由于功能相对单一,治疗效果往往不佳。在此,从中药刺五加中提取得到刺五加源细胞外囊泡(AS-EVs),并将其与聚乙烯亚胺(PEI 600)共同负载到以羧甲基壳聚糖(CMCS)和硫酸软骨素(CS)为基质的可溶性微针中,得到共负载AS-EVs和PEI 600的微针AS-EVs/PEI 600 MN(AP@MN)用于促进糖尿病溃疡伤口愈合。CMCS和CS作为基质具有良好的生物安全性及可降解性,微针能够在伤口处将药物进行精准递送。AS-EVs具有清除活性氧、降低促炎因子表达及促进血管生成作用。PEI 600则能够有效结合伤口中的cfDNA并对其进行清除,同时有效抑制大肠杆菌、金黄色葡萄球菌和MRSA的生长。因此,AP@MN可以实现多种功能,如实现药物精准递送、缓解炎症、促进血管生成、清除cfDNA及抑制细菌生长,在糖尿病感染伤口治疗中展现出巨大的潜力。
iabetic foot ulcers are one of the most common extensions of diabetes. The persistent inflammation and hypoxic environment present in diabetic ulcer wounds can lead to the presence of high expression of inflammatory factors, impaired micro angiogenesis and bacterial infection. Traditional methods are often ineffective in treatment due to their relatively single function. Here, extracellular vesicles (AS-EVs) of Siberian cherry origin were obtained from the traditional Chinese medicine Siberian cherry, and were co-loaded with polyethyleneimine (PEI 600) into soluble microneedles based on carboxymethyl chitosan (CMCS) and chondroitin sulphate (CS), to obtain microneedles AS-EVs/PEI 600 MN co-loaded with AS-EVs and PEI 600 (AP@MN) for promoting diabetic ulcer wound healing. Carboxymethyl chitosan and chondroitin sulfate were used as the substrates with good biosafety and degradability, and the microneedles were able to deliver the drugs precisely at the wound site. AS-EVs had the effects of scavenging reactive oxygen species (ROS), decreasing pro-inflammatory factors expression, and promoting angiogenesis, whereas PEI 600 was able to bind to and scavenge cfDNA in the wounds, and inhibit the growth of Escherichia coli (E.coli), Staphylococcus aureus (S.aureus), and MRSA. Therefore, AP@MN can achieve multiple functions, such as precise drug delivery, inflammation reduction, angiogenesis promotion, cfDNA removal, and bacterial growth inhibition, which shows great potential in the treatment of diabetic infected wounds.
