高血压患者甘油三酯-葡萄糖体质量指数与夜间高血压的相关性分析

背景 ?甘油三酯 - 葡萄糖体质量指数(TyG-BMI 指数)是反映胰岛素抵抗的一种新型指标,已被证实与高血压存在关联,但目前缺乏 TyG-BMI 指数与夜间动态血压参数及夜间高血压的研究。目的 ?探讨 TyG-BMI 指数与夜间动态血压参数及夜间高血压的关系。方法 ?选择2022年6月—2023年10月于华北理工大学附属医院行动态血压监测的原发性高血压患者396例为研究对象,将患者按TyG-BMI指数四分位数分为Q1组(n=99)、Q2组(n=99)、Q3组(n=99)、Q4组(n=99)。收集患者基线资料、实验室检查结果及24小时动态血压监测(ABPM)结果。计算TyG-BMI 指数、甘油三酯 - 葡萄糖(TyG)指数、三酰甘油 / 高密度脂蛋白胆固醇(TG/HDL-C)。应用广义线性模型分析不同 TyG-BMI 指数分组与夜间 ABPM 参数的关联。采用 Spearman 相关性分析 TyG-BMI 指数、TyG 指数、TG/HDL-C 与夜间血压指标相关性。绘制受试者工作特征曲线(ROC 曲线)分析 TyG-BMI 指数对夜间高血压的预测效能并计算 ROC 曲线下面积(AUC)。结果 ?Q1~Q4组年龄、BMI、空腹 TG、空腹血糖(FBG)、肾素 - 血管紧张素 - 醛固酮系统(RAAS)抑制剂、脑卒中、糖尿病、高脂血症比例比较,差异有统计学意义(P<0.05)。Q1~Q4 组全天平均收缩压、全天平均舒张压、日间平均收缩压、日间平均舒张压、夜间平均收缩压、夜间平均舒张压、夜间收缩压下降率、夜间收缩压标准差、夜间舒张压标准差比较,差异有统计学意义(P<0.05)。广义线性模型结果显示 TyG-BMI指数分组是夜间平均收缩压(β=6.623,P=0.002)、夜间平均舒张压(β=4.553,P=0.001)的影响因素;将研究对象按照是否患有高脂血症或糖尿病进行分层,在未患有高脂血症或糖尿病的研究对象中 TyG-BMI 指数分组是夜间平均舒张压(β=8.462,P<0.001)的影响因素;在患有高脂血症或糖尿病的研究对象中 TyG-BMI 指数分组与夜间平均收缩压(β=4.604,P=0.097)、夜间平均舒张压(β=1.988,P=0.285)无相关性。Spearman 相关性分析结果示,TyG BMI 指数与高血压患者的夜间平均收缩压(rs=0.155,P=0.002)呈正相关,TG/HDL-C(rs=0.193,P<0.001)、TyG-指数(rs=0.145,P=0.004)、TyG-BMI 指数(rs=0.250,P<0.001)与夜间平均舒张压呈正相关。绘制 TG/HDL-C、TyG指数、TyG-BMI 指数预测夜间高血压的 ROC 曲线,TG/HDL-C、TyG 指数、TyG-BMI 指数预测夜间高血压的 AUC 分别 为 0.570(95%CI=0.513~0.627,P=0.017)、0.559(95%CI=0.502~0.616,P=0.046)、0.618(95%CI=0.563~0.673,P<0.001);患有高脂血症或糖尿病研究对象中 TG/HDL-C、TyG-BMI 指数预测夜间高血压的 AUC 分别为 0.578(95%CI=0.504~0.651,P=0.043)、0.627(95%CI=0.557~0.698,P=0.001)。结论 ?原发性高血压患者中,TyG-BMI指数与夜间平均舒张压水平存在正相关。TyG-BMI 指数对夜间高血压具有较高的预测价值。
Background??The triglyceride-glucose-body mass indexTyG-BMI indexis a novel indicator reflecting insulin resistance and has been proven to be associated with hypertension. Howeverthere is a lack of research on the relationship between the TyG-BMI index and nocturnal ambulatory blood pressure parameters as well as nocturnal hypertension. Objective ?To explore the relationship between the TyG-BMI index and nocturnal ambulatory blood pressure parameters as well as nocturnal hypertension. Methods ?A total of 396 patients with essential hypertension who underwent ambulatory blood pressure monitoring ABPMat the Affiliated Hospital of North China University of Science and Technology from June 2022 to October 2023 were selected as the study subjects. Patients were divided into four groups based on the quartiles of the TyG-BMI indexQ1n=99Q2n=99Q3n=99and Q4n=99. Baseline datalaboratory test resultsand 24-hour ABPM results were collected. The TyG-BMI indextriglyceride-glucoseTyGindexand triglyceride/high-density lipoprotein cholesterolTG/ HDL-Cratio were calculated. A generalized linear model was used to analyze the association between different TyG-BMI index groups and nocturnal ABPM parameters. Spearman correlation analysis was employed to assess the correlations of the TyG-BMI indexTyG indexand TG/HDL-C with nocturnal blood pressure indicators. Receiver operating characteristicROCcurves were plotted to analyze the predictive efficacy of the TyG-BMI index for nocturnal hypertensionand the area under the ROC curveAUCwas calculated. Results ?There were statistically significant differences in ageBMIfasting TGfasting blood glucoseFBGuse of renin-angiotensin-aldosterone systemRAASinhibitorsand proportions of strokediabetesand hyperlipidemia among the Q1 to Q4 groupsP<0.05. Significant differences were also observed in 24-hour average systolic blood pressure24-hour average diastolic blood pressuredaytime average systolic blood pressuredaytime average diastolic blood pressurenocturnal average systolic blood pressurenocturnal average diastolic blood pressurenocturnal systolic blood pressure dipnocturnal systolic blood pressure standard deviationand nocturnal diastolic blood pressure standard deviation among these groupsP<0.05. The results of the generalized linear model indicated that the TyG-BMI index group was a significant influencer of nocturnal average systolic blood pressure=6.623P=0.002and nocturnal average diastolic blood pressure=4.553P=0.001. When stratified by the presence of hyperlipidemia or diabetesthe TyG-BMI index group was found to be a significant influencer of nocturnal average diastolic blood pressure=8.462P<0.001in subjects without hyperlipidemia or diabetesbut no correlation was observed between the TyG-BMI index group and nocturnal average systolic blood pressure=4.604P=0.097or nocturnal average diastolic blood pressure=1.988P=0.285in subjects with hyperlipidemia or diabetes. Spearman correlation analysis revealed a positive correlation between the TyG-BMI index and nocturnal average systolic blood pressurers=0.155P=0.002in hypertensive patientsand positive correlations between TG/HDL-Crs=0.193P<0.001TyG indexrs=0.145P=0.004TyG-BMI indexrs=0.250P<0.001and nocturnal average diastolic blood pressure. ROC curves were plotted for TG/HDL-CTyG indexand TyG-BMI index to predict nocturnal hypertensionwith AUCs of 0.57095%CI=0.513-0.627P=0.0170.55995%CI=0.502-0.616P=0.046and 0.61895%CI=0.563-0.673P<0.001respectively. In subjects with hyperlipidemia or diabetesthe AUCs for TG/HDL-C and TyG-BMI index predicting nocturnal hypertension were 0.57895%CI=0.504-0.651P=0.043and 0.62795%CI=0.557-0.698P=0.001respectively. Conclusion ?In patients with essential hypertensionthe TyG-BMI index is positively correlated with nocturnal average diastolic blood pressure. The TyG-BMI index has a high predictive value for nocturnal hypertension.
