基于RE-AIM框架的社区认知障碍筛查服务评价指标体系构建研究

背景 我国现已启动老年痴呆防治促进行动，各地围绕社区老年人群积极开展认知功能筛查工作，然而当前尚无适宜的评价标准可作为社区开展认知障碍筛查工作的考核依据。目的 基于RE-AIM框架构建适用于社区的认知障碍筛查服务评价指标体系，为各地开展大规模社区老年人群认知功能筛查项目的考核评价和质量控制提供参考。方法 通过文献研究和政策分析，围绕RE-AIM框架的五大要素拟建指标体系初稿，于2023年10月—2024年1月遴选15名专家开展德尔菲专家咨询构建社区认知障碍筛查服务评价指标体系，并采用乘积法确定指标权重。结果 两轮咨询的专家积极系数为100%和93.3%，专家权威系数为0.83和0.86，专家意见协调系数为0.242和0.265（P<0.001），最终确立的指标体系包含5个一级指标、15个二级指标、20个三级指标。一级指标及权重分别为筛查可及性（0.203 5）、筛查有效性（0.203 5）、组织采纳性（0.194 8）、筛查实施性（0.203 5）、筛查持续性（0.194 8）。结论 本研究初步探索并构建的社区认知障碍筛查服务评价指标体系，包含5个一级指标、15个二级指标、20个三级指标，具有较好的科学性和可靠性，对于完善认知障碍筛查考核体系具有一定参考价值。
BackgroundChina has now launched the Alzheimer's disease prevention and treatment promotion actionand local communities are actively carrying out cognitive function screening around the community elderly populationhoweverthere is no suitable evaluation standard to be used as the basis for the screening of cognitive impairment in the community. ObjectiveTo construct an evaluation system for cognitive impairment screening services applicable to the communityand to provide a reference for the appraisal and quality control of large-scale community-based cognitive function screening programs among older adults in various regions of China. MethodsBy literature research and policy analysisthe first draft of the indicator system was proposed around the five elements of the RE-AIM frameworkand 15 experts were selected to carry out a Delphi expert consultation to construct an evaluation indicator system for community-based cognitive impairment screening services from October 2023 to January 2024. The weights of the indicators were determined using the multiplier method. ResultsThe positive coefficients of experts in the two rounds of Delphi were 100% and 93.3%the experts' authority coefficients were 0.83 and 0.86and the coordination coefficients of experts' opinions were 0.242 and 0.265P<0.001 and the finalized evaluation system consists of 5 first-level indicators15 second-level indicatorsand 20 third-level indicators. ConclusionThe evaluation index system of the community cognitive impairment screening service initially explored and constructed in this studywhich contains 5 first-level indicators15 second-level indicatorsand 20 third-level indicatorsis of good scientific validity and reliabilityand has certain reference value for improving the assessment system of cognitive impairment screening.
