摘要

目的 将柯氏模型与行动研究相联合,全面评价小儿外科专科护士培训效果,并通过行动研究不断的完善专科护士培训的方案,以期提高培训质量。方法 以行动研究法为框架,将行动研究法运用于小儿外科专科护士培训班,通过计划、行动、观察、反思螺旋式循环,构建小儿外科专科护士培训班;以柯氏模型为指导,从反应层、学习层、行为层与结果层4个层面运用问卷调查法、访谈法等依次对培训效果进行相关评价,对评价进行反思、总结、整改,进行下一轮的行动研究。结果 行动研究小组构建了基于柯式模型的评价模式,从专科护士的培训效果评价,包括参考模型确立、评价内容确定、评价方法设计、评价标准建立和评价模式实际运行等方面开展研究,共实施了3期培训班。实施结果显示,反应层评价显示三期学员对培训满意度均有较高的满意度,且满意度逐年提高;学习层评价显示三期学员的理论成绩评分均高于培训前(P<0.05);行为层评价显示临床带教老师和学员均认为培训后,学员的专科护士行为态度及岗位能力均有所提高;结果层评价显示学员在培训后期专科护士的核心能力均有较高的提升。结论 行动研究法是解决临床教学问题的有效方法,以柯式模型为依据评价模式评价内容全面,有助于促进培训成果应用,引导培训计划修正,可以提高专科护士培训质量。

Abstract

Objective This study aims to integrate the Kirkpatrick model with action research to comprehensively evaluate the effectiveness of pediatric surgical specialty nurse training. Additionally, through continuous action research, we seek to refine the training program for specialty nurses in order to enhance training quality.Methods Utilizing action research as a framework, this study applies it within pediatric surgical specialty nurse training programs. Through a spiral cycle of planning, acting, observing, and reflecting, we construct the training program for pediatric surgical specialty nurses. Guided by the Kirkpatrick model, evaluations are conducted at four levelsreaction, learning, behavior, and resultsusing methods such as questionnaires and interviews. Each evaluation is followed by reflection and summary leading to corrective actions for subsequent rounds of action research.Results The action research team developed an evaluation model based on the Kirkpatrick framework. Research was conducted on various aspects of evaluating the effectiveness of specialty nurse training including establishing reference models, determining evaluation content, designing evaluation methods, setting standards for assessment criteria, and implementing practical evaluation models across three cohorts of trainees. Results indicated that at the reaction level all three cohorts reported high satisfaction with their training experience which improved year-on-year; at the learning level scores from theoretical assessments were significantly higher post-training (P < 0.05); at the behavior level both clinical instructors and trainees noted improvements in trainees attitudes towards specialized nursing practices and job competencies following completion of their courses; finally at the results level there was a notable enhancement in core competencies among trainees during later stages post-training.Conclusion Action research proves to be an effective method for addressing issues in clinical education. The comprehensive nature of content evaluated using a Kirkpatrick-based model facilitates better application of training outcomes while guiding necessary adjustments in future educational plans thereby improving overall quality in specialty nurse training.
