炎症反应介导内皮功能障碍及血脑屏障损伤在脑微出血中的作用
脑微出血是脑小血管病的典型影像学表现,其特征性脑小血管受损导致的含铁血黄素沉积,已被证实与认知功能障碍、脑出血事件、缺血性卒中复发及出血转化显著相关。然而,由于其发病机制不清导致临床实践中仍缺乏有效的早期干预和治疗方案。近年来研究表明,炎症反应、内皮功能障碍和血脑屏障通透性增加可能协同参与了脑微出血的病理发展过程,其为疾病的靶向干预和治疗提供了新的研究方向。本文综述了炎症反应介导内皮功能障碍及血脑屏障损伤在脑微出血发病机制中的作用以及相关临床研究证据,以期为脑微出血的诊疗提供参考。
Cerebral microbleeds are a typical imaging manifestation of cerebral small vessel disease, characterized by the deposition of hemosiderin resulting from damage to small cerebral vessels. They have been confirmed to be significantly associated with cognitive dysfunction, intracerebral hemorrhage events, recurrence of ischemic stroke, and hemorrhagic transformation. However, due to unclear pathogenic mechanisms, there is still a lack of effective early intervention and treatment options in clinical practice. Recent studies have suggested that inflammatory responses, endothelial dysfunction, and increased blood-brain barrier permeability may collaboratively participate in the pathological development of cerebral microbleeds, providing new research directions for targeted intervention and treatment of the disease. This article reviews the role of inflammatory response-mediated endothelial dysfunction and blood-brain barrier damage in the pathogenesis of cerebral microbleeds, along with relevant clinical research evidence, in order to provide reference for the diagnosis and treatment of cerebral microbleeds.
