摘要
本病例报告聚焦1例43岁宫颈癌放疗后并发放射性膀胱炎、阴道炎、输尿管梗阻（伴肾后性肾功能不全），同时合并重度贫血、肺转移、呼吸衰竭及血栓病史的复杂患者，突显多并发症交织下中西医结合护理的实践价值与挑战。报告系统阐述以多学科协作为基础，围绕泌尿系感染控制、梗阻解除、皮肤黏膜修复及心理干预四大核心问题展开的护理过程，涵盖评估（CTCAE 4.0放射性皮炎分级、实验室指标监测）、诊断、计划、实施与评价。通过西医重症监护、抗感染、液体管理与中医复方黄柏洗液湿敷、耳穴压丸等技术融合，患者10天后外阴破溃愈合、排尿通畅，肾功能恢复（肌酐降至89μmmol/l），未发生脓毒症等严重并发症。本案例表明，基于精准评估的个体化中西医结合护理模式能有效优化宫颈癌放疗后多重并发症患者的结局，为同类病例的综合管理提供可操作的实践经验。
Abstract
This case report focuses on a complex 43-year-old patient with radiation cystitis, vaginitis, and ureteral obstruction (complicated with post-renal renal insufficiency) after cervical cancer radiotherapy, who also had a history of severe anemia, lung metastasis, respiratory failure, and thrombosis. It highlights the practical value and challenges of integrated traditional Chinese and Western medicine (TCM-WM) nursing in the context of multiple concurrent complications. The report systematically elaborates the nursing process based on multidisciplinary collaboration, focusing on four core issues: control of urinary tract infection, relief of obstruction, repair of skin and mucous membranes, and psychological intervention, covering assessment (CTCAE 4.0 radiation dermatitis grading, laboratory index monitoring), diagnosis, planning, implementation, and evaluation. Through the integration of Western medical intensive care, anti-infection therapy, fluid management with TCM external treatments such as compound Phellodendri Chinensis Cortex lotion wet compress and auricular point pressing, the patients vulvar ulcer healed, urination became smooth, renal function recovered (creatinine decreased to 89 mmol/L) within 10 days, and no serious complications such as sepsis occurred. This case indicates that the individualized TCM-WM nursing model based on precise assessment can effectively optimize the outcomes of patients with multiple complications after cervical cancer radiotherapy, providing operable practical experience for the comprehensive management of similar cases.关键词
