不同频率振腹推拿干预糖耐量异常大鼠胰岛素抵抗的量效研究

摘要
目的:探讨400次/min、500次/min、600次/min振腹推拿对糖耐量异常(IGT)大鼠糖代谢及胰岛素抵抗(IR)的改善作用,并明确其优势频率。方法:将30只SD大鼠分为正常组、IGT组、400振腹组、500振腹组和600振腹组,每组6只。采用高脂饮食联合小剂量链脲佐菌素建立IGT模型,各振腹组每日于神阙穴接受相应频率干预20分钟,连续7天。检测空腹血糖(FPG)、餐后2小时血糖(2hPG)、空腹胰岛素(FINS)并计算胰岛素抵抗指数(HOMA-IR),观察胰腺病理形态。结果:与正常组相比,IGT组FPG、2hPG、FINS及HOMA-IR均显著升高(P<0.05),胰岛结构受损。振腹干预后,各频率组指标均有改善,其中400振腹组FPG显著降低(P<0.05),FINS与HOMA-IR恢复至近正常水平,胰岛形态改善最明显;600振腹组体质量增长受控(P<0.05);500及600振腹组改善作用相对有限。结论:振腹推拿可改善IGT大鼠糖代谢异常与IR,且具有频率依赖性,400次/min为优势频率,其机制可能与增强胰岛素敏感性有关。
Abstract
Objective: To investigate the effects of abdominal vibration massage at frequencies of 400, 500, and 600 times/min on glucose metabolism and insulin resistance (IR) in rats with impaired glucose tolerance (IGT), and to identify the optimal frequency. Methods: Thirty SD rats were divided into a normal control group, an IGT model group, and three vibration groups (400, 500, and 600 times/min), with 6 rats per group. The IGT model was established using a high-fat diet combined with a low-dose streptozotocin injection. The vibration groups received daily 20-minute interventions at the corresponding frequency on the Shenque (CV8) acupoint for 7 consecutive days. Fasting blood glucose (FPG), 2-hour postprandial blood glucose (2hPG), and fasting insulin (FINS) levels were measured, and the homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. Pancreatic histopathology was also observed. Results: Compared with the normal group, the IGT group showed significant increases in FPG, 2hPG, FINS, and HOMA-IR (P<0.05), along with islet structural damage. After vibration intervention, all frequency groups exhibited varying degrees of improvement. The 400 times/min group showed a significant reduction in FPG (P<0.05), with FINS and HOMA-IR returning to near-normal levels, and demonstrated the most pronounced improvement in islet morphology. The 600 times/min group showed controlled body weight gain (P<0.05). The improvements in the 500 and 600 times/min groups were relatively limited. Conclusion: Abdominal vibration massage can improve glucose metabolism abnormalities and IR in IGT rats, with its effects being frequency-dependent. A frequency of 400 times/min appears to be the optimal parameter in this study, potentially exerting its core regulatory effect by enhancing insulin sensitivity.关键词
