摘要
背景 甘油三酯(TG)水平升高与心血管疾病之间存在长期联系。在接受他汀类药物治疗且低密度脂蛋白胆固醇(LDL-C)水平控制良好的患者中,TG水平升高的患者患动脉粥样硬化性心血管疾病的风险高于TG水平较低的患者。因此,及早发现与高甘油三酯血症(HTG)进展相关的因子对于HTG相关疾病的防治意义重大。而硫氧还蛋白相互作用蛋白(TXNIP)在脂质代谢中起着关键的作用。目的 评估血清TXNIP水平与脂耐量减低(IFT)及HTG的相关性。方法 选择2019年2月—2020年2月河北省人民医院体检中心受检者235例为研究对象,均进行口服脂肪耐量试验(OFTT),根据OFTT结果将受检者分为脂耐量正常组(NFT组,75例)、IFT组(85例)和HTG组(75例);并根据血清TG四分位数将受试者分为Q1~Q4组。检测空腹血糖(FBG)、2 h口服葡萄糖耐量试验(OGTT)血糖、空腹胰岛素(FINS)、血清尿酸(SUA)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)、空腹及高脂餐后4 h总胆固醇(TC)、TG、高密度脂蛋白胆固醇(HDL-C)、LDL-C。计算非HDL-C(Non-HDL-C)和富含TG的脂蛋白残留物(TRLRs),评估稳态模型胰岛素抵抗指数(HOMA-IR)及胰岛β细胞功能指数(HOMA-β)。采用酶联免疫吸附试验测定血清TXNIP,分析TXNIP与各指标的相关性。结果 IFT组和HTG组血清TXNIP水平较NFT组升高,且HTG组高于IFT组(P<0.05);Q2、Q3、Q4组TXNIP水平高于Q1组,Q3、Q4组TXNIP水平高于Q2组,Q4组TXNIP水平高于Q3组(P<0.05)。血清TXNIP水平与SUA、FBG、FINS、HOMA-IR、HOMA-β、0 hTC、4 hTC、0 hTG、4 hTG、0 hLDL-C、4 h LDL-C、Non-HDL-C、TRLRs、ApoB呈正相关,与0 hHDL-C、4 hHDL-C呈负相关(P<0.05)。校正主要混杂因素后,多元线性回归分析结果显示,IFT和HTG是血清TXNIP水平的独立影响因素(IFT:β=0.184,P=0.006;HTG:β=0.441,P<0.001)。结论 血清TXNIP水平与脂耐量减低程度呈正相关,降低血清TXNIP可能会延缓HTG相关疾病的进展。
Abstract
BackgroundThere is a long-term link between elevated triglyceride TG levels and cardiovascular diseases.In patients receiving statin therapy and with LDL-C levels well controlledthose with elevated TG levels still have a higher risk of developing atherosclerotic cardiovascular disease than those with lower TG levels.Thereforethe early detection of factors related to the progression of HTG is essential for ASCVC prevention and treatment. And thioredoxin-interacting proteinTXNIPplays a crucial role in lipid metabolism.Objective To evaluate the association of serum TXNIP levels with impaired fat toleranceIFT and hypertriglyceridemiaHTG. MethodsA total of 235 volunteers with different fat tolerances were selected for the oral fat tolerance testOFTT. According to the OFTT resultsthe subjects were divided into a normal fat tolerance groupNFT group75 casesan IFT group 85 casesand an HTG group 75 cases. Serum TXNIP levels were positively correlated with SUAFBGFINSHOMA-IRHOMA-0 h TC4 h TC0 h TG4 h TG0 h LDL-C4 h LDL-CNon-HDL-CTRLRsand ApoBand negatively correlated with 0 h HDL-C and 4 h HDL-C P<0.05.After adjusting for major confoundersmultiple linear regression analysis showed that IFT and HTG were significantly correlated with TXNIP levelsIFT=0.184P=0.006HTG=0.441P<0.001. ConclusionSerum TXNIP levels were positively correlated with the degree of impaired fat toleranceand decreasing serum TXNIP levels may delay the progression of HTG-related diseases.关键词
