摘要

炎症性肠病(Inflammatory Bowel Disease, IBD)是一种以慢性、复发性肠道炎症为特征的复杂性疾病,其发病机制涉及遗传、免疫、环境及肠道菌群等多种因素。近年来,肠道上皮屏障功能障碍被广泛认为是IBD发生发展的核心病理环节,屏障结构的破坏可导致肠腔抗原异常易位,触发并持续放大免疫炎症反应。尾型同源盒转录因子2(Caudal-related homeobox 2, CDX2)作为肠道特异性转录因子,在肠上皮细胞分化、增殖及稳态维持中发挥关键的"主调控因子"作用。现有证据表明,CDX2通过精准调控紧密连接蛋白、黏附连接核心组件以及黏液蛋白MUC2来维持肠上皮的结构与功能完整。本综述旨在系统剖析CDX2在稳态维持与IBD发展中的双重功能,特别探讨其在肠道微环境下通过干细胞修复的分子间相互作用。这篇综述不仅有助于肠道稳态调控网络的理解,为更进一步开发以"主动修复屏障"为核心的新型IBD治疗靶点与精准策略提供了理论支撑。

Abstract

Inflammatory Bowel Disease (IBD) is a complex disorder characterized by chronic, relapsing intestinal inflammation, with a pathogenesis involving a combination of genetic, immunological, environmental, and gut microbiota factors. In recent years, intestinal epithelial barrier dysfunction has been widely recognized as a core pathological link in the onset and progression of IBD. The disruption of the barrier structure can lead to the abnormal translocation of luminal antigens, thereby triggering and continuously amplifying the immune-inflammatory response. As an intestine-specific transcription factor, Caudal-related homeobox 2 (CDX2) plays a crucial "master regulator" role in the differentiation, proliferation, and homeostasis maintenance of intestinal epithelial cells. Emerging evidence indicates that CDX2 maintains the structural and functional integrity of the intestinal epithelium by precisely regulating tight junction proteins, core components of adherens junctions, and the mucin protein MUC2. This review aims to systematically analyze the dual functions of CDX2 in homeostasis maintenance and IBD progression, with a particular focus on its intermolecular interactions during stem cell-mediated repair within the intestinal microenvironment. This review not only deepens the understanding of the intestinal homeostasis regulatory network but also provides theoretical support for the development of novel IBD therapeutic targets and precision strategies centered on "active barrier repair.
