学校周边绿地环境与苏州市学龄儿童及青少年超重/肥胖的关联性研究

研究目的:近年来,学生群体超重与肥胖发生率仍呈持续上升趋势。本研究旨在探讨学校周边绿化程度与学生超重/肥胖之间的关联关系。研究方法:选取 2020-2021 年苏州市 793 所学校共 1005935 名学生为研究对象。采用归一化植被指数(NDVI)与土壤调整植被指数(SAVI)评估绿地暴露水平;基于中国高分辨率大气污染物数据集获取大气污染数据;运用广义线性混合模型分析学校周边绿化与学生超重/肥胖的关联。研究结果:在500m、1000m、1500m缓冲区范围内,归一化植被指数(NDVI)每增加一个四分位距(IQR),学生超重风险均显著降低;对应的比值比及95%置信区间分别为0.965(0.948~0.982)、0.972(0.956~0.988)、0.977(0.960~0.994)。大气污染物在NDVI与超重/肥胖的关联中存在一定中介效应。采用SAVI指数分析所得结论与NDVI基本一致;开展亚组分析及剔除初一新生后,研究结果仍具有稳健性。研究结论:学校周边绿化环境对防控学生超重、肥胖具有积极保护作用,且大气污染物在二者关联中发挥一定中介作用。为保障青少年身体健康,应进一步增加学校周边绿化暴露空间。
Abstract
Aim: The rates of overweight and obesity among students have still been continuously increasing in recent years. This study aimed to explore the association between greenness around schools and overweight/obesity in students. Methods: 1005935 students from 793 schools in Suzhou during 2020-2021 were obtained. Normalized difference vegetation index (NDVI) and soil adjusted vegetation index (SAVI) were used to evaluate the greenness exposure. We used the China High Air Pollutants dataset to obtain air pollution data. Generalized linear mixed models were used to explore the relationship between school greenness and overweight/obesity. Results: The results indicate that an increase in NDVI by IQR in the 500m, 1000m, and 1500m buffer zones is associated with a reduced risk of overweight. The corresponding OR (95%CI) is 0.965 (0.948-0.982), 0.972 (0.956-0.988), and 0.977 (0.960-0.994) respectively. Air pollutants played a certain mediating role in the relationship between NDVI and overweight/obesity. The results were similar in analysis using SAVI, and the results remained robust when subgroup analyses were performed or first-year students were excluded. Conclusions: The greenness surrounding schools has a beneficial impact on students\' overweight and obesity, and air pollutants played a certain mediating role in this relationship. We ought to create more greenness exposure around schools for students\' health.关键词
