以提升基层全科医师岗位胜任力为导向的情境模拟教学实践专家共识（2026）

摘要
当前我国基层全科医学教学仍存在实践场景受限、训练标准化不足等局限性，难以高效提升基层全科医师岗位胜任力，无法充分满足基层医疗卫生服务对复合型全科人才的核心需求。情境模拟教学是弥补临床实践教学短板、强化基层全科医师综合诊疗能力的重要手段。为此，专家组系统梳理国内外相关文献，以2023版WONCA树胜任力框架为理论依据，经多轮专家咨询与修订完善，制订本共识。本共识明确以基层全科医师岗位胜任力为导向的情境模拟教学内涵，构建“症状诊疗与临床决策、慢性病管理、基层急重症处置、社区健康管理与促进”四大阶梯式教学模块，建立PEARL-link六步法标准化实施流程，形成基层全科医师、指导医师、课程体系三维质量评估体系，并完善师资保障、案例质量、伦理安全等全链条质量控制机制。本共识实现教学内容、实施流程、质量评价、保障体系一体化设计，可为基层全科医学情境模拟教学的规范化、同质化开展提供参考，助力提升基层全科医学人才培养质量。
Abstract
At present, the teaching of general practice at the primary care level in China remains constrained by limited practical clinical scenarios and insufficient standardization of training, which impede the effective enhancement of the post competency of primary care general practitioners (GPs) and fail to adequately meet the core demand of primary healthcare services for versatile, compound-type general practice talents. Scenario-based simulation teaching serves as a critical modality to compensate for the deficiencies of clinical practice teaching and to reinforce the comprehensive diagnostic and therapeutic capabilities of primary care GPs. To address this, an expert panel systematically reviewed relevant domestic and international literature, adopted the 2023 version of the WONCA Tree competency framework as the theoretical foundation, and developed the present consensus through multiple rounds of expert consultation, deliberation, and iterative refinement.This consensus explicitly defines the connotation of scenario-based simulation teaching oriented toward the post competency of primary care GPs, and establishes four progressive teaching modules: symptomatic diagnosis and clinical decision-making, chronic disease management, emergency management in primary care, and community health promotion and management. Furthermore, it formulates a standardized six-step implementation procedure, designated as the PEARL-link method, and develops a three-dimensional quality evaluation system encompassing primary care GPs, supervising instructors, and the curriculum system. In addition, a comprehensive quality control mechanism is established, covering faculty assurance, case quality, and ethical safety across the entire teaching chain.By achieving the integrated design of teaching content, implementation procedures, quality evaluation, and support systems, this consensus provides a reference for the standardized and homogenized delivery of scenario-based simulation teaching in primary care general practice, thereby contributing to the improvement of talent cultivation quality in this field.关键词
