抗血管紧张素Ⅱ 1型受体自身抗体在原发性肾小球疾病患者血清中的表达及意义
he role of angiotensin Ⅱ type 1 receptor activating antibody in the primary glomerular disease
目的 探讨原发性肾小球疾病患者血清中抗血管紧张素Ⅱ1型受体自身抗体(angiotensin Ⅱ type 1 receptor activating antibody,AT1-AA)的表达及其临床意义。 方法 采用酶联免疫吸附试验(ELISA),对326例原发性肾小球疾病患者与同期收集的197例健康对照者进行血清AT1-AA的检测。将原发性肾小球疾病患者分为AT1-AA阴性组和AT1-AA阳性组,比较两组的临床和病理指标。 结果 ①原发性肾小球疾病患者血清中AT1- AA阳性率为35.58%(116/326),明显高于正常对照组的9.64%(19/197)(P<0.01);②原发性肾小球疾病患者中有免疫抑制剂用药史的患者AT1-AA阳性率明显低于无用药史的患者(P<0.05);③原发性肾小球疾病患者中无高血压组和有高血压组的AT1-AA阳性率均显著高于正常对照组(P<0.01),但无高血压组和有高血压组之间AT1-AA阳性率的差异并无统计学意义(P>0.05);④AT1-AA阳性组中女性患者所占比例、血清球蛋白水平高于AT1-AA阴性组(P<0.05)。 结论 原发性肾小球疾病患者血清中存在AT1-AA的高表达,提示AT1-AA可能参与了原发性肾小球疾病的发病机制;非恶性高血压与AT1-AA之间无明显相关性;女性中出现抗AT1受体自身抗体的可能性高于男性;免疫抑制剂可以降低抗AT1受体自身抗体的阳性率。
Objective:To investigate the expression and clinical significance of serum angiotensin Ⅱ type 1 receptor activating antibodies (AT1-AA) in patients with primary glomerular disease. Methods:Clinical and pathologic data were collected in patients with primary glomerular disease (n=326).AT1-AA was detected in serum samples by enzyme-linked immunosorbent assay (ELISA). Results:①The positive rates of AT1-AA in patients with primary glomerular disease were 35.58%,much higher than those of healthy donors(9.64%)(P<0.01).②Immunosuppressants can inhibit the production of AT1-AA. ③In patients with primary glomerular disease, there were no significant differences between those who had hypertension and those who hadn't hypertension (P>0.05). But they all had significantly higher level of positive incidences than those of healthy donors.④Compared with AT1-AA-negative patients,the AT1-AA-positive patients had significantly higher level of serum globulin and the rate of femal (P<0.05). Conclusion:There is high expression of AT1-AA in patients with primary glomerular disease. The instance of primary glomerular disease may be related with autoantibodies against AT1-receptor, which may be relerant with the level of serum globulin and the rate of female patients. There are no relevance between non-malignant hypertension and AT1-AA. Immunosuppressants can effect the expression of AT1-AA.
