1例PICC中线导管应用在减重手术患者中的护理体会

肥胖患者的血管壁容易堆积脂肪,血管也会被脂肪包围和压迫,导致血流阻力增加,血管不够顺畅,血管壁弹性降低。肥胖患者术后禁饮食不仅需长达3天的补液、抑酸等静脉治疗,还要进行早期活动促进快速康复,因此,静脉通路不仅要满足临床快速补液的需求,还要在活动过程中维持通畅。鉴于上述特点,为肥胖症患者选择一种有效的静脉补液通道极其重要。美国静脉输液护理学会(INS) 2021年输液治疗实践标准提出采用塞丁格或改良后塞丁格技术置管,可以提高穿刺成功率,同时减少并发症。在肥胖患者的减重手术中使用中线导管,既能满足长期静脉输液治疗的需要,又能减轻患者痛苦保护血管,减轻护士频繁穿刺的压力。置入中线导管后,无需进行X线定位,减少了辐射损伤和患者治疗费用。
Obese patients tend to accumulate fat on their blood vessel walls, which can also be surrounded and compressed by fat, leading to increased blood flow resistance, less smooth blood vessels, and reduced wall elasticity. Postoperative fasting in obese patients not only requires intravenous therapy such as fluid replacement and acid suppression for up to 3 days, but also early activity to promote rapid recovery. Therefore, venous access should not only meet the clinical needs for rapid fluid replacement, but also maintain patency during the activity process. Given the above characteristics, it is extremely important to choose an effective intravenous fluid replacement channel for obese patients. The American Society of Venous Infusion Nursing (INS) 2021 infusion treatment practice standard proposes the use of Sedinger or modified Sedinger technique for catheterization, which can improve puncture success rate and reduce complications. The use of midline catheters in weight loss surgery for obese patients can not only meet the needs of long-term intravenous infusion treatment, but also alleviate patient pain, protect blood vessels, and alleviate the pressure of frequent punctures by nurses. After inserting a midline catheter, there is no need for X-ray localization, reducing radiation damage and patient treatment costs.
