64层螺旋CT儿童鼻窦低剂量扫描
Multidetector CT of the Paranasal Sinus in Children: Optimal Strategies of Radiation Dose Reduction
目的 探索64层螺旋CT低剂量扫描在儿童鼻窦中的应用,获得不同参数的辐射剂量并对图像质量进行评估。方法 东芝64排螺旋CT,分别选择管电压为120kV和100kV时,转速0.5s/rot,螺距41不变,用80mA、40 mA、30 mA、20 mA、10mA的剂量对1具儿童尸体头颅标本鼻窦进行扫描,比较不同参数时的图像质量。选取能满足临床需求的最低辐射剂量作为扫描参数。并用该参数对33例儿童鼻窦炎患者进行扫描。评价图像的质量和显示精细结构的能力。结果 当mA从100mA降到30mA时,辐射剂量明显下降,保持30mA不变,100kV时有效CT剂量指数(CTDIvol)明显小于120kV(3.1mGy<4.8mGy)。当mA降至20mA以下,图像质量明显变差。用100kV,30mA进行33例儿童鼻窦扫描,鼻窦内各结构显示清晰,所有图像均满足临床需求。CTDIVOL平均为3.03±0.09(mGy),剂量长度乘积(DLP)为30.63±2.58(mGy.cm),有效辐射剂量为0.07±0.003(mSv),仅为常规剂量的1/10。结论 64层螺旋CT当100kV, 30mA(15mAs)扫描儿童鼻窦时,可能是满足临床要求的最低剂量。
Objective Our objective was to show that in low-dose MDCT of the sinuses in children the effective dose can be lowered to a level comparable to that used for standard radiographic images, with resultant CT scans that are still of diagnostic image quality. Methods With the same protocol (0.5s/rot and pitch 41), a pediatric cranium specimen were scanned with 120kV and 100kV respectively (Toshiba Aquilion 64-row spiral CT) at different mA (80,40,30,20,10). The CT dose index and images were compared among the different mA groups. The minimum kV and mA value was determined. Thirty-three children were examined to test the validity of the minimum kV and mA value. Results The CT radiation dose was significantly reduced when mA from 80mA to 10mA. With the same 30mA, the CT radiation dose of 100kV was obvious less than 120kV (3.1mGy<4.8mGy). The minimum kV and mA value was 100kV and 30 mA. Thirty-three children were examined with 100kV, 30mA. All images were measured up and in accord with clinic results. The average CTDIVOL and DLP were 3.03±0.09(mGy),30.63±2.58 (mGy.cm), and the effective radiation dose was 0.07±0.003(mSv),which was only 1/10 compared to the routine radiation dosage. Conclusion The CT radiation dose would be the lowest in diagnostic image quality for pediatric para-nasal sinus scanning at 100kV, 30mA (15mAs) with Toshiba Aquilion 64-row spiral CT.
