三维CT评价支抗种植体和磨牙的移位
Movement of miniscrews and molars for anchorage under orthodontic force:3-dimensional computed tomography registration evaluation
目的:尽管微种植支抗被许多学者和临床医生称之为绝对支抗,但是施力后微种植支抗可否发生移位尚不清楚。本实验旨在利用三维CT探讨正畸治疗过程中微种植支抗的位置变化。方法:60例正畸治疗患者,上牙槽前突,UD-SN、U1-SN大于正常一个标准差,轻度拥挤,正畸需拔除第一前磨牙,以种植支抗辅助内收切牙。正畸治疗前、后,分别拍摄头颅CT、三维重建,确定微种植体、磨牙、切牙的位置,通过前后CT图像重叠配准,分析微种植体、磨牙、切牙治疗前后的移位程度。测量数据应用SAS9.13软件包进行配对t检验。结果:微种植体头部近中移动0.23mm,尾部近中移动0.23mm,上颌第一磨牙牙冠近中移动0.92mm,牙根近中移动0.91mm,上颌中切牙切嵴和根尖处分别内收5.94mm和1.40mm,伴有上颌中切牙1.84mm内收。结论:在种植支抗辅助正畸治疗中,微种植体和磨牙均发生了不同程度的近中移动,选择偏近中的位点植入微种植体可能有利于种植支抗的稳定和组织安全。
Purpose Although miniscrews have been used as absolute anchorage for a long time, their behavior under Orthodontic loading is still unclear clinically. Therefore, this study was designed to evaluate the behavior of miniscrews under loading by retrospective 3-dimensional registration. Methods Sixty adult patients who had miniscrews as anchorage for en-masse retraction of anterior teeth were studied. Computerized tomography scans were made before force application and after closure of the extraction spaces, respectively. The 3-dimensional reconstruction and registration of before and after computerized tomography data were performed to assess the displacement of the miniscrews,first molars, and maxillary central incisors. Results The miniscrews and the maxillary first molars drifted mesially 0.23 and 0.91mm apically, and 0.23 and 0.92mm coronally; the amounts of maxillary incisor retraction at the edge and the apex were 5.94 and 1.40mm, respectively, with 1.84mm of maxillary central incisor intrusion. Conclusions Our results indicated that the miniscrews and the maxillary first molars were mesially displaced under orthodontic loading. A mesial site for miniscrews might be a better choice for long-term stability.
