脑卒中住院患者不同时间点营养状况影响因素分析

背景脑卒中患者常发生营养不良,既往研究大多关注其入院时营养状况,对住院及出院时营养状况研究较少,若能动态观察并分析其影响因素,则能改善卒中患者营养状况及预后。目的分析脑卒中患者不同时间点(入院、7d、出院)营养状况的影响因素。方法本次研究共纳入177例患者,进行一般资料、营养状况、吞咽状况及神经功能缺损程度收集,分析各因素与脑卒中患者营养状况的关系。结果单因素分析结果显示:两组患者入院时营养状况在年龄、吸烟饮酒史、营养支持方式、WST评分方面差异具有统计学意义(P<0.05);7d时在年龄、卒中后肺炎、营养支持方式、洼田饮水试验(WaterSwallowTest,WST)评分、NIHSS评分方面差异具有统计学意义(P<0.05~0.001);出院时在营养支持方式、WST评分、NIHSS评分方面差异具有统计学意义(P<0.05~0.001);多因素Logistic回归分析显示,WST评分(OR=2.041,95%CI:1.354-3.076)为患者入院时营养不良预测因素;年龄(OR=1.029,95%CI:0.994-1.065)、营养支持方式及NIHSS评分(OR=1.108,95%CI:1.029-1.194)为7d时营养不良预测因素;营养支持方式为出院时营养不良独立预测因素。结论WST评分是患者入院时营养不良的影响因素,年龄、营养支持方式及NIHSS评分为7d时营养不良影响因素;营养支持方式为出院时营养不良影响因素;

Background Stroke patients often suffer from malnutrition. Most of the previous studies focused on their nutritional status at the time of admission, but there is a lack of studies on the nutritional status during different periods of hospitalization. Objective To investigate the influencing factors of nutritional status in stroke patients at different time points(admission,7d,discharge) .Methods According to the inclusion and exclusion criteria, a total of 177 patients were included in this study. General information, nutritional status, swallowing status and degree of neurological deficit were collected, and the relationship between each factor and the nutritional status of stroke patients was analyzed.Results The results of univariate analysis showed that the nutritional status of the two groups of patients at admission was significantly different in terms of age, smoking and drinking history, and nutritional support methods (P<0.05);There were statistically significant differences in age, post-stroke pneumonia, nutritional support,Water Swallow Test score, and NIHSS score at 7 days (P<0.05-0.001); logistic regression analysis of nutritional support at discharge showed that WST score (OR=2.041, 95) %CI: 1.354-3.076) was a statistically significant difference in the way of predicting malnutrition, WST score and NIHSS score at admission (P<0.05~0.001);Multivariate Logistic regression analysis showed that WST score (OR=2.041,95%CI1.354-3.076 was a predictor of malnutrition at admission;Age (OR=1.029,95%CI:0.994-1.065), nutritional support method, and NIHSS score (OR=1.108,95%CI:1.029-1.194) were predictors of malnutrition at 7d; nutritional support was independent predictors of malnutrition at discharge. Conclusion WST score is the influencing factor of malnutrition at admission, age, nutritional support mode and NIHSS score are malnutrition at 7d; nutritional support is malnutrition at discharge;
