健康老年人肠道定植产ESBLs大肠埃希菌中插入序列共同区(ISCR)研究
ISCR in extended-spectrum β-lactamases producing Escherichia coli
目的:调查至华山医院体检健康老年人肠道定植产ESBL大肠埃希菌中常见插入序列共同区(ISCR)阳性率及ESBL的基因型。方法:对我院体检健康老年人肠道分离大肠埃希菌,采用微量稀释法行药敏,检测菌株耐药性情况。应用PCR方法,确认ISCR阳性率及ESBL的基因型。依据是否含ISCR序列,将上述菌株分为两组,比较这两组细菌对各种抗生素的耐药率。结果:在262株肠杆菌科细菌中,基因阳性率分别为ISCR1(22.9%)、ISCR2 (10.3%)、ISCR3(5.3%)、ISCR4(5.3%)、ISCR5(1.9%)、CTX-M-1(34.3%)、CTX-M-9 (69.5%)、CTX-M-2(2.3%)、SHV(0.38%)、TEM(41.98%)。各种抗生素的耐药率如下:阿米卡星(0.76%)、头孢噻肟(99.6%)、磷霉素(3.4%)、头孢吡肟(12.98%)、头孢他啶(22.9%)、头孢西丁(9.9%)、左氧氟沙星(46.6%)、氨苄西林(100%)、氨苄西林舒巴坦(57.6%)、哌拉西林他唑巴坦(0%)、美罗培南(0%)、厄他培南(0%)。除美罗培南、厄他培南及哌拉西林他唑巴坦这三种耐药率为0的抗生素外,含ISCR菌株对大多数抗生素耐药率均比不含ISCR者高(阿米卡星除外)。 结论:ISCR可能在细菌耐药性增高中发挥重要作用。
Objective: To investigate the prevalence of ISCR and the genotype of ESBL among the ESBL-producing Escherichia coli isolated from healthy elderly. Methods: From January to March in 2015, a total of 262 Escherichia coli were isolated. The production of extended-spectrum β-lactamases(ESBLs) was initially screened by phenotypic experiments.PCR was used to detect the gene of ISCR1, ISCR2, ISCR3, ISCR4, ISCR5, CTX-M-1,CTX-M-9,SHV,TEM, CTX-M-2.Antimicrobial susceptibility test was performed by Agar dilution method according to the most recent guidelines of the Clinical and Laboratory Standards Institute. These strains were divided into ISCR-positive and ISCR-negative groups. We compared the antibiotic resistance rate of the two groups. Result: For the 262 strains, the antimicrobial resistance rate was as follows: Amikacin(0.76%),Cefotaxime(99.6%),Fosfomycin(3.4%),Cefepime(12.98%),Ceftazidime(22.9%),Cefoxitin(9.9%),Levofloxacin(46.6%),Ampicillin(100%),Ampicillin-Sulbactam(57.6%),Piperacillin-Tazobactam(0%),Meropenem(0%) and Ertapenem(0%). The positive rate of the genes were ISCR1 (22.9%), ISCR2 (10.3%), ISCR3 (5.3%), ISCR4 (5.3%), ISCR5 (1.9%), CTX-M-1 (34.3%), CTX-M-9 (69.5%), SHV (0.38%), TEM (41.98%), CTX-M-2 (2.3%). The group containing ISCR had higher resistance rate in most antibiotics, except for Amikacin and others with the resistance rate of zero. Conclusion: ISCR may be associated with higher antibiotic resistance in ESBLs producing Escherichia coli.
