目的:基于推拿对周围神经损伤的良好临床疗效,从实验研究角度探寻推拿对周围神经损伤后所出现的感觉及运动功能障碍的改善效应,并进一步揭示推拿对周围神经损伤后的中枢修复机制。方法:通过对推拿干预坐骨神经损伤大鼠的脊髓切片进行HE染色和免疫组化染色,观察脊髓切片组织形态学和神经元胞浆内胶质细胞源性神经营养因子(GDNF)的表达变化,为揭示推拿促进神经修复的机制提供理论支持。结果:通过脊髓切片HE染色发现,治疗组大鼠脊髓中细胞排列在完整性、方向性方面好于模型对照组和模型组,说明推拿对周围神经损伤后脊髓腹角运动神经元的再生具有一定的促进作用;染色后脊髓神经元胞浆内的GDNF出现阳性表达,治疗组脊髓标本中GDNF的平均光密度要高于模型组和模型对照组,从而证明了推拿是通过增加脊髓GDNF含量来促进神经再生的结论。结论:综合脊髓HE染色、脊髓免疫组化染色,可初步推断,推拿可促进坐骨神经损伤大鼠感觉及运动功能恢复,作用途径为对脊髓背角感觉神经元的营养与保护,对脊髓腹角运动神经元再生的支持与促进,起效机理之一为促进GDNF在脊髓中的表达。

【Objective】This study is based on the good clinical effect of Tuina treatment in peripheral nerve injuries, in order to confirm the ameliorative effects of Tuina treament on the sensory disturbance and motor dysfunction caused by peripheral nerve injuries, meanwhile interpret the central mechanism of Tuina on the restoration of peripheral nerve injury. 【Methods】Selecting the sciatic nerve injured rats treated by Tuina treatment, observing the spinal cord which are stained with HE staining by the optical microscope; analyzing the spinal cord which are stained with immunohistochemical staining by the the optical microscope, the glial-cell-line derived nerotrophic factor(GDNF) is chosed as the basic point from neuro trophic factors which can promote the nerve regeneration, to provide theoretical support for revealing the the central mechanism of Tuina treament on the restoration of peripheral nerve injury. 【Results】In view of HE staining of spinal cord, the integrity and the directivity of the spinal cord cell arrangement in Tuina treatment group is better than model group and control group, cell nuclear is locating in the center, cell regeneration can be seen, axons are extending to the distal part, that is indicating that Tuina treatment can promote the regeneration of spinal cord anterior horn motor neurons after peripheral nerve injuries. The GDNF in cytoplasm of spinal cord neurons has positive expression after stained,it is the more uniform brown granules, the average optical density of GDNF in Tuina treatment group is higher than model group and control group, thus improving that Tuinatreament can promote the nerve regeneration by increased the content of GDNF in spinal cord.【Conclusion】Comprehensive analyze the result of histomor phology and immunohistochemical staining, according to preliminary inference, the mechanism ameliorative effects of Tuina treament on the sensory disturbance and motor dysfunction caused by peripheral nerve injuries is promoting the expression of GDNF in spinal cord.
