目的 : 观察过度通气对早期基因:早期生长因子(Egr-1)、C-jun 和促炎因子IL-1β mRNA、蛋白表达影响及意义。方法: 选择清洁级雄性SD大鼠32只随机分为4组(n=8),戊巴比妥钠腹腔麻醉,小型动物呼吸机给予:VT: 45ml/Kg , R:40次/min ,I:E:1:2 ; PEEP =0 , 室内空气吸入。于机械通气30、60、90、120min各取肺组织分别以RT-PCR、免疫组化和HE染色方法检测Egr-1、C-jun 、IL-1 mRNA及蛋白表达及肺组织病理改变。结果: 1、机械通气30min时 Egr-1、C-jun 、IL-1βmRNA即有表达,90min达高峰持续至120min,较30min有显著差异(P<0.05)。2、免疫组化:30min时三种蛋白表达不显著,60min时Egr-1、C-jun以胞浆为主,IL-1β略有表达;90-120min,胞浆、胞核染色阳性细胞数增多,IL-1β表达亦增加,较30min时有显著意义(P<0.01)。3、光学显微镜发现:90min肺泡壁略增厚、出血、肺泡腔内渗出和炎症细胞增多,120min加重。结论 : 机械通气启动和上调早期基因及促炎因子表达,为机械通气肺损伤的早期预测、发病机制研究以及进行基因干预或靶疗提出新的方向。

bjective :To observe the expression of early response genes due to injurious tidal volume ventilation in adult rat lung in vivo . Methods : 32 SD male rats were randomly divided into four groups (n=8). The animals were anesthetized by intraperitoneal (ip) injection of 35 mg/kg pentobarbital and placed in dorsal decubitus throughout the experiment.After tracheostomy a 14-gauge cannula was inserted into the trachea. The rats were ventilated at a VT of 45 ml/kg, respiratory rate (RR) of 40 breaths/min, positive end-expiratory pressure (PEEP) of zero cmH2O, and inspired the room air with a ventilator for small animals. Lung tissues were obtained at 30 、60、90、120 min respectively during ventilaition.and then to detect the expression of mRNA of Egr-1, C-jun , IL-1β and the proteins by RT-PCR and immunohistochemical technique ; as well as the Histologic assessment were performed . Results : At 30min ventilation caused the genes mRNA upregulated of Egr-1、C-jun 、IL-1β , but not with
