同型半胱氨酸与心脑血管疾病的关系及致病机理
Pathogenesis of Hyperhomocysteinemia
目的:综述同型半胱氨酸 (Hcy) 的代谢及心脑血管疾病的致病机理。方法:本文对近几十年的研究结果进行分析,对Hcy的代谢,致病机理以及高同型半胱氨酸血症 (HHcy) 的预防进行综述。结果:Hcy是蛋氨酸代谢的中间产物,是一种含硫的氨基酸,在心脑血管疾病的发病机制中起重要作用,Hcy通过参与氧化反应产生超氧化物及过氧化物损伤血管内皮细胞,改变凝血因子功能,引起血管壁巨噬细胞反应,造成血管壁脂肪堆积等,结论:Hcy是心脑血管病的一个新的独立的危险因素。
Increased plasma homocysteine (Hcy) is a significant and independent risk factor for cardiovascular disease (CVD). It can cause multi-disease manifestations such as: smooth muscle proliferation, premature occlusive vascular disease, progressive arterial stenosis, haemostatic changes, placental vasculopathy, spontaneous early abortion, birth defects, impaired cognitive function and dementia. The aim of this review paper was to summarize the role of elevated Hcy levels in cardiovascular and other diseases and the molecular mechanisms and pathophysiology involved in deleterious manifestations of hyperhomocysteinemia (HHcy). We have collected data from MEDLINE, Current Contents and scientific journals, which included 131 publications from 1932 to 2007. The pathophysiology could be caused by the direct toxicity of Hcy on tissues. Mechanisms through which Hcy induces CVD include endothelial dysfunction, oxidative stress, smooth muscle cell (SMC) proliferation, enhancing thrombogenicity, formation of reactive oxygen species (ROS), hypomethylation, induction of unfolded protein response (UPR) and extracellular matrix modification. Furthermore, we enumerated some existing therapeutic approaches. However, further studies should be conducted to investigate new operative therapeutic methods of HHcy.
