目的:探讨计算机辅助设计技术在颏部水平截骨整形中的应用,研究计算机辅助技术在小颏畸形治疗中的精确性和优越性。 方法:术前应用计算机辅助设计技术,设计手术方案,确定颏部水平截骨前移以及下降量,并制作个性化导板,指导手术操作,对9例小颏畸形患者进行颏部水平截骨前移术。通过对比术前设计和术后的头颅三维重建模型,评估设计的精确性。 结果:所有病例术后效果明显,外观满意,颏部畸形得到了明显矫治。 结论:本研究立足于临床,将计算机辅助技术和小颏畸形的治疗紧密结合,应用计算机辅助设计技术的优势,精确指导颏部水平截骨整形术,并利用计算机辅助制造技术制作相应个性化导板,精准指导手术操作,节约手术时间,降低手术风险。术后手术效果满意,外形及功能改善良好,术后随访疗效确切,证明计算机辅助技术应用于颏部水平截骨整形术是一种值得推广的方法。

Objective: To explore the application of computer aided design (CAD) and computer aided manufacturing (CAM) in horizontal advancement genioplasty, and study the accuracy and superiority of CAD and CAM in the treatment of microgenia. Methods: The technique of CAD was applied to make the individual operation plan, determine the vertical and sagittal movements, prefabricate the individualized. With CAM, surgical templates were made to guide the surgery. According to preoperative planning, 9 patients with microgenia were received horizontal advancement genioplasty. To estimate the precision of CAD and CAM, we compared the preoperative design with the postoperative CT image. Results: All 9 patients suffering from microgenia were satisfied with the effective outcomes and the deformity of microgenia was corrected obviously. Conclusion: Based on clinic, our study combines the computer-assisted techniques with the treatment of microgenia, to fully take advantage of CAD and CAM to make the surgical plan and individualized templates, which can guide the surgery, save the operation time and reduce the risk of surgery. The aesthetic outcomes showed that applying computer-assisted techniques for horizontal advancement genioplasty provided precise surgical result.
