胸腰椎爆裂骨折相邻椎间盘组织病理学改变的对比分析

目的:通过观察胸腰椎爆裂骨折后上下位椎间盘组织学形态改变以及重要炎性介质及下游产物在软骨终板内的表达情况,比较上、下位椎间盘的损伤程度,初步探讨创伤后椎间盘一系列改变的可能发生机制。方法:收集24例胸腰椎爆裂骨折患者的病例资料及损伤椎体的相邻椎间盘组织,组织学处理后进行1. HE染色。根据组织学的表现来评价椎间盘退变程度。2.阿辛蓝染色比较受损椎体上下椎间盘髓核内蛋白多糖的含量3.对软骨终板进行白细胞介素-1 (IL-1β), 诱导性一氧化氮合成酶 (iNOS)的免疫组化染色,观察其在软骨终板组织内的表达特点。结果:1.采用椎间盘退变评分标准,爆裂骨折邻近上下位椎间盘没有明显的统计学差别。2. 胸腰椎爆裂骨折患者中下位椎间盘髓核中的蛋白多糖含量要高于上位椎间盘髓核组织3. 胸腰椎爆裂骨折患者中上位椎间盘软骨终板内白细胞介素-1以及诱导性一氧化氮合成酶表达阳性率均高于下位椎间盘。结论:1.通过对胸腰椎爆裂骨折上下相邻椎间盘比较:上位椎间盘在创伤后受损较相应下位椎间盘更明显。2. 传统的退变椎间盘评价系统可能不适用于对创伤后椎间盘组织学改变的相关评估。3.创伤后椎间盘改变的相关机制可能有别于单纯椎间盘退变。
objective to compare the injury extents of superior and inferior disc tissues of burst fracture and discuss the possible pathologic mechanism of the post-traumatic disc changes. Methods to collect the superior and inferior disc tissues in 24 patients during operation . HE, Alcian blue staining, Immunohitochemistry(IHC) were performed. Experimental data were analysed with Photoshop8.0, Image-Pro Plus and SPSS13.0. Results By using immunohistochemistry IL-1β was expressed in the matrix around the end-plate cartilage cells and iNOS was expressed in the cartilage cells. superior disk end-plate are more apt to injury in burst fracture. injuties in superior disc adjacent to the involved vertebra was more severe the content of proteoglycan and decreased sharply in superior discs compared to inferior discs(p=0.005,p=0.04<0.05). Conclusions In thoracolumbar burst fracture, By molecular pathologic measurement the superior disk end-plate are more apt to injury in burst fracture. As the different expressions of the two proteins between the degenerative disk and injury disk were concerned, we suspect that there exist some different pathologic mechanism while the disk was injuried.
