全科住院医师规范化培训中师资教学目标认知和制定现况质性研究

背景 教学目标是教学的灵魂和支柱,与课堂教学效果密切相关。目前针对全科住院医师规范化培训(全科住培)中师资对教学目标认知及制定方面的研究较少。目的 选取国内优秀全科医学师资,以教学查房为场景进行深入访谈,了解师资对教学目标认知和制定依据、建构、撰写等方面内容,探讨与院校教育不同、专业性强、重在岗位胜任力培养的全科住培如何通过教学目标更好促进教学。方法 选取所在地区全科医学发展好、工作地点为国内知名医学院附属医院全科医学科、科室为国家级全科住培基地,且从事全科住培带教5年及以上、副主任医师及以上、获得院级及以上教学荣誉、目前仍参与教学查房并在教学活动中制定教学目标的优秀全科医学师资为研究对象,于2022年2月至6月,通过线上视频会议对其进行一对一的半结构化深度访谈。应用 NVivo 12 软件,采用主题框架分析法对资料进行整理、分析,并提炼访谈主题。结果 共入选8名全科医学师资,通过对访谈资料进行分析,共提炼出3个主题和四个子主题:1)全科师资能较好掌握教学目标制定的原则;2)教学目标制定缺乏制度保障,包括两个子主题:缺乏理论基础、缺乏规范制定环境;3)全科师资对教学目标在岗位胜任力培养中的作用认知不足,包括两个子主题:对教学目标存在认知缺陷、制定的教学目标对教学活动指导意义不大。结论 即使优秀全科医学师资,在教学目标认知和制定上仍存在问题,一定程度上影响教学效果。应重视和保障教学目标有效制定,加强相关理论培训,以教学目标为抓手促进全科医学师资队伍同质化发展和建设,以培养出更多合格的全科医生。

Background Teaching objectives are the soul and pillar of teaching activity and are closely related to the effect of teaching effects. Currently, there are few studies on the teachers’ cognition and formulation of teaching objectives in the standardized training of residency general practitioners (general practice residential training). Objectives To select outstanding domestic general practice teachers, to conduct in-depth interviews in the scene of teaching ward rounds, to understand teachers’ cognition, formulation basis, construction, writing, and other aspects of teaching objectives, to discuss how to better promote teaching through teaching objectives in the general practice residency training that is different from the college education and highly professional and focuses on job competency training. Methods The study subjects were outstanding general practice teachers who are in the region with good development of General Practice discipline, whose workplace is the General Practice department in an affiliated hospital of a well-known medical school in China and the department is a national residential training base for general practitioners, being the deputy chief physician or above, teaching General Practice for 5 years or above, have won related teaching honors on a college level or above, and are still participating in teaching ward rounds and setting teaching objectives in such teaching activities. From February to June 2022, the one-on-one semi-structured in-depth interviews were conducted through online video conferences. Using the NVivo 12 software, the topic frame analysis method was used to organize and analyze the data and to refine the interview topics. Results Totally 8 General Practice teachers were enrolled. Through the analysis of the interview data, totally 3 themes and four subthemes were extracted: 1) general practice teachers can grasp the principles of teaching objectives setting; 2) teaching objectives setting lacks institutional guarantees, including two sub-themes: the lack of theoretical foundation and the lack of normative setting environment; 3) general practice teachers have insufficient cognition of the role of teaching objectives in the cultivation of post competency, including two sub-themes: cognitive deficiencies in teaching objectives, and teaching objectives setting has little significance in guiding the teaching process.Conclusions Even the outstanding general practice teachers still have some problems in the cognition and formulation of teaching objectives, which may affect the teaching quantity and quality. It is necessary to strengthen and ensure the effective formulation of teaching objectives and to enhance relevant theoretical training. Meanwhile, taking teaching objectives as the starting point is to promote the homogeneous development of general practice teachers, so as to cultivate more qualified general practitioners.
