目的:研究类风湿关节炎(RA)患者外周血及关节液Dickkopf-1(DKK-1)水平和血清抗酒石酸酸性磷酸酶5b(TRAP5b)活性在骨破坏及骨代谢失衡中的意义。方法:采用酶联免疫吸附法(ELISA)检测230例RA患者、50例骨关节炎(OA)患者及150例健康对照血清DKK-1水平及和关节液DKK-1的浓度及血清TRAP5b的活性浓度,比较各组DKK-1及TRAP5b水平及其与临床相关指标的相关性。结果:(1) RA患者血清DKK-1水平明显高于健康对照组(P<0.001)和OA患者组(P<0.001)。(2) RA关节液DKK-1水平明显高于OA(P=0.007),15例配对血清和关节液DKK-1水平呈显著正相关(r=0.860, P<0.0001)。(3) RA骨质疏松组血清TRAP5b活性显著高于非骨质疏松组 (P<0.001)。 (4) RA患者外周血DKK-1水平及TRA5b活性均与病程及Sharp评分正相关(P<0.05)。结论:RA患者外周血DKK-1和TRAP5b与局部骨破坏密切相关,可作为骨破坏及骨代谢失衡的评价指标。

Objectives To investigate the roles of circulating Dickkopf-1(DKK-1) and tartrate-resistant acid phosphatase 5b (TRAP5b) in RA patients with bone destruction and osteoporosis. Methods Serum samples were obtained from 230 RA, 50 osteoarthritis (OA) and 150 health controls. Synovial fluid (SF) was collected from 45 RA and 15 OA patients, and 15 paired sera were collected from these RA patients. The level of DKK-1 and TRAP5b were measured by ELISA. Results Serum DKK-1 of RA patients were significantly higher than OA and health controls (P<0.001); SF DKK-1 was significantly higher in RA than in OA (P=0.007), which was correlated with paired sera in RA(r=0.860,P<0.0001); There was obvious discrepancy of TRAP5b in RA patients with and without osteoporosis(P<0.001). However, such discrepancy wasn't displayed in serum DKK-1 level (P>0.05); Serum DKK-1 and serum TRAP5b activity was positive correlated with disease duration and Sharp score(P<0.05). Conclusion Circulating DKK-1 and TRAP5b activity were closely related to systemic bone metabolism imbalance and local bone destruction in patients with RA.
