胃癌中maspin和突变型p53蛋白的表达及临床病理学意义

目的:采用免疫组织化学方法检测maspin蛋白与p53蛋白的表达,分析maspin蛋白表达与胃癌临床病理特征的关系及其与p53蛋白表达的相关性,旨在探讨其在胃癌发生发展中的生物学意义及相关分子病理学机制。方法:检测胃癌标本156例(其中105例距癌灶边缘大于5cm处取配对正常胃黏膜作为对照),肠上皮化生106例及异型增生6例中maspin蛋白与p53蛋白的表达。结果:maspin在胃癌组织(51.3%),肠上皮化生(95.3%)以及正常胃粘膜(0.0%)中表达差异有显著性(p<0.001)。Maspin蛋白在肠型胃癌中和弥漫型胃癌,各期胃癌中,在淋巴结转移组中和无淋巴结转移组中表达无显著差异(p>0.05)。在胃癌组织中maspin蛋白表达与p53蛋白的表达无等级相关(p>0.05)。但肠型胃癌中maspin蛋白与p53蛋白的spearman相关系数为-0.06,提示存在负相关趋势。结论:在从肠上皮化生到肠型胃癌的过程中,maspin表达的失活可能起重要作用。在胃癌中,maspin蛋白与突变型p53蛋白的表达无明显等级相关,但在肠型胃癌中,二者之间存在负相关趋势。Maspin在胃癌发生和转移中的确切作用以及作用的分子生物学机制还有待研究。
Objective: To detect the expressions of maspin and mutant p53 protein, and to analyze the correlation of maspin protein expression with pathological typing, staging of gastric carcinoma, lymphnode metastasis and the expression of mutant p53 protein so as to investigate its biological significance and related molecular pathological mechanism in the carcinogenesis and progress of gastric carcinoma. Methods: PV9000 immunohistochemical method was used to detect the expression of maspin and p53 in156 cases of tumor specimens and 105 cases of matched normal gastric mucosa and , 106 intestinal metaplasia (IM), 6 dysplasia (Dys). Results: There were significant differences of maspin expression among the gastric cancer tissues (51.3%), intestinal metaplasia(95.3), and normal gastric mucosa. There were no statistical difference of the positive rates of maspin protein expression in intestinal type and diffuse type gastric carcinoma, different stages of gastric carcinoma, with and without lymph node metastasis. The rank correlation detection revealed that there was no rank correlation between expressions of maspin and p53 protein in gastric carcinoma tissues(P>0.05), while spearman rank correlation coefficient was -0.06 in gastric carcinoma of intestinal type, indicating that there was a negative correlation tendency. Conclusions:Inactivation of maspin expression may play an important role during the progress of intestinal metaplasia transforming to the gastric carcinoma. In gastric carcinoma, the expression of maspin expression had on evident rank correlation with that of mutant p53 protein,while in intestinal-type gastric carcinoma, negative correlation tendency occurs between them. Further studies are needed on the certain role and molecular biological mechanism of maspin in the carcinogenesis and metastasis of gastric carcinoma.
