甲状腺功能正常的高血压患者血清甲状腺激素水平与高尿酸血症的相关性研究
【摘要】 背景 血尿酸(SUA)与高血压的发生发展密切相关,高血压患者常伴随甲状腺功能改变。但目前在甲状腺功能正常的高血压患者中,甲状腺激素水平与高尿酸血症(HUA)患病风险的相关研究鲜有报道。目的 探讨甲状腺功能正常的原发性高血压(EH)患者血清甲状腺激素水平与HUA的相关性。方法 回顾性选取2019年1月至2020年12月延安大学咸阳医院心血管内科收治的甲状腺功能正常的EH患者267例作为研究对象。根据是否合并HUA,分为非HUA组166例,合并HUA组101例。比较2组间的基本资料、SUA、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平,并进行Pearson相关分析与多因素Logistic回归分析。结果 合并HUA组男性占比、体质指数(BMI)、舒张压(DBP)、吸烟人数、饮酒人数、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、血肌酐(Scr)、SUA、FT3水平均高于非HUA组,年龄低于非HUA组(均P<0.05)。多因素Logistic 回归分析显示,男性较女性更易患HUA,BMI、TG、LDL-C、FT3、Scr水平升高与年龄是EH患者发生HUA的独立影响因素[OR(95% CI) 2.843 (1.121~7.215),1.126(1.020~1.234),1.824(1.300~2.560),2.804(1.157~6.795) ,2.297(1.326~3.977) ,1.071(1.041~1.102),0.959(0.931~0.989)]。Pearson相关分析显示,EH患者FT3水平与SUA呈正相关(r=0.327,P<0.001)。结论 合并HUA的甲功正常的EH患者血清FT3水平较无HUA患者高,血清FT3水平升高与EH患者发生HUA密切相关,应引起临床医生重视。
bstractBackground Serum uric acid (SUA) is closely related to the occurrence and development of hypertension. Hypertensive patients are often accompanied by changes in thyroid function. However, there are few reports on the correlation between thyroid hormone levels and the risk of hyperuricemia (HUA) in hypertensive patients with normal thyroid function. Objective To investigate the correlation between serum thyroid hormone levels and HUA in euthyroid patients with essential hypertension (EH). Methods 267 EH patients with normal thyroid function admitted to Xianyang Hospital of Yan'an University from January 2019 to December 2020 were retrospectively selected as the study subjects. According to the combination of HUA or not, 166 cases were divided into non-HUA group and 101 cases were combined with HUA group. The basic information, SUA, thyroid stimulating hormone (TSH), free triiodothyronine (FT3) and free thyroxine (FT4) levels were compared between the two groups, and Pearson correlation analysis and multivariate Logistic regression analysis were performed. Results The proportion of males, body mass index (BMI), diastolic blood pressure (DBP),number of smokers, number of drinkers, total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), FT3, serum creatinine (Scr) and SUA levels in HUA group were higher than those in non-HUA group, and the age was lower than that in non-HUA group (all P< 0.05).Multivariate Logistic regression analysis showed that males were more prone to HUA than females, and elevated BMI, TG, LDL-C, FT3, Scr levels and decreased age were independent influencing factors for HUA in hypertensive patients [OR(95%CI) 2.843 (1.121~7.215), 1.126 (1.020 ~ 1.234), 1.824 (1.300 ~ 2.560), 2.804 (1.157 ~ 6.795), 2.297 (1.326 ~ 3.977), 1.071 (1.041 ~ 1.102), 0.959 (0.931 ~ 0.989)]. Pearson correlation analysis showed that FT3 level was positively correlated with SUA in hypertensive patients (r = 0.327, P<0.001).
