欠发达地区全科医生岗位胜任力的评价指标研究
Study on evaluation indexes of general practitioners' post competence in Underdeveloped Areas
目的 构建用于评判欠发达地区全科医生岗位胜任力的指标体系,为欠发达地区全科医学人才培养、考核、评价提供参考依据。方法 于2021年1月至2021年6月,采用目的抽样法,分别从熟悉全科医学领域前沿、从事全科医学实践和行政管理、东西部对口帮扶曾在欠发达地区挂职的全科医生等人员中遴选咨询专家18位。运用文献研究法、德尔菲法和层次分析法完成欠发达地区全科医生岗位胜任力的评价指标筛选和各指标权重的确定。结果 建立了4个一级指标、20个二级指标、44个三级指标的欠发达地区全科医生岗位胜任力评价指标,一级指标医疗服务能力、公共卫生服务能力、组织管理和服务协调能力、职业素养的权重分别为0.3509、0.1091、0.1891、0.3509。二级指标中排名前四位分别为业务素质、学习思维能力、沟通协调、常见病多发病诊治。三级指标中排名前四位的分别为文明沟通友好交流、人文关怀、临床适宜技术、常见病多发病诊断和治疗。结论 本研究构建了欠发达地区全科医生岗位胜任力的评价指标,可以为欠发达地区全科医生岗位胜任力评价体系的持续质量改进提供科学依据。
Objective To construct an index system to evaluate the post competency of general practitioners and to provide reference for the training, assessment and evaluation of general practitioners in underdeveloped areas. Methods From January to June of 2021,by purposive sampling method18 consultant experts were selected respectively from the following 3 types of personnel:who were familiar with the frontier theory of general medicine,engaged in general medical practice and administrative management,and helped the general practitioners in underdeveloped areas. The methods of literature research, Delphi and analytic hierarchy process were used to evaluation indexes of general practitioners' post competency and to determine the weight of each index in underdeveloped areas.Results The competency index system of general practitioners in underdeveloped areas has been established, which includes 4 first grade indexes, 20 second grade indexes and 44 third grade indexes.The weights of the first grade indexes including medical service capacitypublic health service capacityorganization management and service coordination abilityand Professionalism were 0.35090.10910.1891 and 0.3509 respectively.The top 4 combined weights of the second grade indexes were professional quality, learning and thinking ability, communication and coordination, diagnosis and treatment of common diseases.The top 4 combined weights of the third grade indexes were civilized communication and friendly exchange, humanistic care, clinical appropriate technology, diagnosis and treatment of common diseases.Conclusion This study constructs the evaluation index of general practitioners' post competency in underdeveloped areas, which can provide scientific basis for the continuous quality improvement of general practitioners' post competency evaluation system in underdeveloped areas.
