目的探讨糖酵解过程中的关键酶1型和2型乳酸脱氢酶激酶(PDK)以及A型乳酸脱氢酶(LDH)在甲状腺乳头状癌(PTC)及正常甲状腺组织中的表达特点,并进一步探讨其与BRAF基因突变的关系。方法应用免疫组化染色及BRAF基因PCR测序方法,检测了150例PTC及100例正常甲状腺组织中PDK1,2及LDH-A的表达情况;用基因测序方法对上述79例组织中的BRAF基因进行检测。结果 PTC组织中PDK1、PDK2和LDH-A的阳性表达率分别为76%、81%和95%,正常组织中对应蛋白的阳性表达率分别为20%、16%和30%。PTC中上述蛋白的阳性表达率明显高于正常组织(都有P<0.001)。PTC中PDK1,2和LDH-A的蛋白表达评分均明显高于癌旁正常组织(都有P<0.001)。甲状腺癌组织中PDK1阳性表达组BRAF基因突变率明显高于PDK1阴性表达组(P<0.001)。PDK2和LDH-A阳性表达与BRAF基因突变之间无明确相关关系。PDK1表达阳性组淋巴结转移率显著高于PDK1表达阴性组(P<0.05)。结论 PTC的发生可能与糖酵解通路中的PDK1的过表达有关。BRAF基因突变可能通过促进糖酵解通路中的关键酶如PDK1表达的变化参与甲状腺癌的发生发展。

im To explore the expression pattern of pyruvate dehydrogenase kinase-1 (PDK1), PDK2 and lactate dehydrogenase-A (LDH-A) in papillary thyroid carcinoma (PTC), and to explore the relationship of the expression of these key enzymes and BRAF mutation in PTC. Method The expression of PDK1, PDK2 and LDH-A were examined in 150 PTC tissues and 100 normal thyroid tissues using immunohistochemical staining. BRAF mutation was examined in 79 above PTC tissues using PCR sequencing. Results The positive rate of PDK1, PDK2 and LDHA in PTC tissues was 76%, 81% and 95%, respectively. The positive rate in normal thyroid tissues was 20%, 16% and 30%, respectively, which were lower than those in PTC tissues (P<0.001). The expression scores of PDK1, PDK2 and LDHA were higher in PTC than those in normal thyroid tissues (P<0.001). The positive expression of PDK1 was associated with BRAF mutation in PTC tissues (P<0.05). The rate of lymphatic metastasis was higher in group with positive PDK1 than that in group with negative PDK1 (P<0.05). Conclusion The overexpression of PDK1 may contribute to the growth of PTC cells through activation of glycolysis. BRAF mutation may change the expression of the key enzymes involving in glycolytic pathway and promote the development of PTC.
