目的 探讨2型糖尿病(T2DM)伴发高血压患者血清视黄醇结合蛋白(RBP4)的水平变化,及RBP4与脂代谢,胰岛素抵抗等的关系。 方法 入选对象180例,其中正常对照组50例、糖尿病无并发症组50例、糖尿病伴发高血压组80例。检测其空腹血清RBP4、空腹血糖(FBG)、身高、体重、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、计算体重指数(BMI)和稳态模型评估法胰岛素抵抗指数(HOMA-IR)。分析三组RBP4水平,及其与上述其他指标的相关性。 结果 正常对照组、糖尿病无伴发症组与糖尿病伴高血压组间,RBP4水平逐步升高;年龄、TC、TG、SBP、DBP,在糖尿病两亚组间存在差异,与正常对照组无差异;糖尿病无并发症组BMI、HbA1c、HOMA-IR高于正常对照组,与糖尿病伴发高血压组无差异。HDL-C、LDL-C三组间无统计学差异。多元线性回归分析显示,在糖尿病无并发症组中,HbA1c 为RBP4影响因素;糖尿病伴发高血压组,HOMA-IR为RBP4的影响因素。结论 在糖尿病伴发高血压病变者中RBP4 水平升高,HbA1c 、HOMA-IR可能为RBP4的影响因素。

Objective To investigate the serum retinol-binding protein (RBP4) levels change in type 2 diabetes (T2DM) patients with hypertension ,associated with serum retinol-binding protein (RBP4) levels change, and the relationship between with RBP4 and fat metabolism, insulin resistance relationship. Method Selected object 180 cases, including normal control group of 50 patients, 50 Diabetes patients without complications of diabetes,and 80 Diabetic cases associated with hypertension . Detect the fasting serum RBP4, fasting blood glucose (FBG), height, weight, fasting insulin (FINS), glycosylated hemoglobin (HbA1c), systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), body mass index (BMI) and homeostasis model assessment insulin resistance index method (HOMA-IR). Analysis of three groups of RBP4 levels, and with the other indicators of relevance. The results of the normal control group, diabetic complications unaccompanied group between the two groups with hypertension and diabetes, RBP4 levels gradually increased; Age, TC, TG, SBP, DBP, in diabetes there are differences between the two sub-groups, with no difference between the normal control group ; diabetes without complications group BMI, HbA1c, HOMA-IR higher than the control group, associated with hypertension and diabetes were no different. HDL-C, LDL-C was no significant difference among the three groups. Multiple linear regression analysis showed no complications in the diabetic group, HbA1c of RBP4 factors; diabetes associated with hypertension, HOMA-IR as RBP4 factor. Conclusion Serum levels of RBP4 is increased in Diabetic associated with hypertension, HbA1c, HOMA-IR may be the factors affecting the RBP4.
