人巨细胞病毒在Rasmussen脑炎中的表达分析

目的:探究人巨细胞病毒(HCMV)与Rasmussen脑炎(Rasmussen's Encephalitis, RE)发生发展的相关性。方法:利用免疫组化染色方法,检测RE患者手术脑组织标本中HCMV特异性蛋白pp65的分布,与对照组相对比,分析HCMV感染或携带与RE发病的相关性。结果:7例RE患者除去1例患者行活检手术外,对全部26例行半球手术的RE患者脑组织的石蜡包埋切片进行免疫组化和原位杂交试验,免疫组化结果显示HCMV-pp65抗原在其中23例中阳性表达,阳性率88.46%,对比正常对照组结果,阳性结果具有统计学意义。结论:RE的发生与HCMV感染具有相关性。HCMV作为诱发因素,参与了RE的发病起始和疾病的病程进展,但没有参与直接攻击中枢神经系统,没有直接导致RE病损区的病理变化。HCMV作用于RE病损区的神经元胞膜和突起,调节宿主细胞的代谢,未直接影响细胞核,未直接改变细胞的理化性质。
Object: To study the relation between human cytomegalovirus (HCMV) infection or carrier and the occurence of Rasmussen's Encephalitis (RE) patients. Method: Brain tissue of RE group and control group were studied by pp65 immumohistochemical staining. By statistic analysis, compared the result of two groups. Result: Form April 2008 to December 2013, The epilepsy center of Beijing Sanbo Brain Hospital performed surgeries on 27 patients with RE (RE group, n = 27), autopsy center of Beijing University offered 6 cases of postmortem brain tissue specimens without epilepsy or nervous system diseases, and no poison death (normal control group, n = 6). Immunohistochemical results showed that positive expressions of HCMV-pp65 antigen in 23 cases, with positive rate 88.46%, which was significantly higher than the control group. Conclusion: HCMV infection is associated with the occurrence and progress of RE as a trigger factor, but no attack CNS and no involved in the pathological changes directly HCMV infections act on the membrane and processes of neurons.
