N-乙酰半胱氨酸治疗D-氨基半乳糖诱导猪急性肝衰竭对血氨、内毒素和细胞炎症因子的影响研究

摘要] 目的 探讨N-乙酰半胱氨酸治疗D-氨基半乳糖诱导猪急性肝衰竭对肝脏功能和炎症因子的影响; 方法 随机将14只雌性中国实验小型猪分为急性肝衰竭组(6只)和药物治疗组(8只);急性肝衰竭组和药物治疗组分别应用D-氨基半乳糖(1.2g/kg)静脉滴注制备药物性急性肝衰竭动物模型;正常对照组未给予任何处理,药物治疗组给予N-乙酰半胱氨酸治疗,急性肝衰竭组同期给予相同剂量的生理盐水;分别采用测定动物血氨、血浆内毒素以及血清中肿瘤坏死因子-α、白介素-1β含量。结果 药物治疗组和急性肝衰竭组相比,血氨和炎症细胞因子TNF-α、IL-1β水平在24和48时间有明显改善,内毒素水平在12小时有明显改善,但12小时后没有显著性差别:血氨水平(24小时:112.5±21.7vs95.4±25.9,P<0.05;48小时:257.5±50.7vs 213.6±70.5,P<0.05)、内毒素水平(12小时:0.165±0.05 vs0.13±0.01,P<0.05);TNF-α和IL-1β水平在24、48时间点有明显下降,TNF-α水平(24小时:24.0±5.3vs19.3±4.2,P<0.05;48小时:29.2±8.7 vs 26.1±3.8,P<0.05)、IL-1β(24小时:18.5±3.6vs15.2±3.4,P<0.05;48小时:24.3±6.3vs20.1±5.2,P<0.05)。结论 NAC可改善D-氨基半乳糖诱导的猪急性肝衰竭肝脏功能,使血氨血症和内毒素血症得到一定改善,可抑制炎症反应的快速发展,稳定机体的内环境。
Objective To study changes of serum ammonia, endotoxin and inflammatory factor of N-acetylcysteine on D-galactosamine-induced acute hepatic failure in pigs. Methods Sixteen minipigs were divided into two groups at random: normal control group (n=2), acute failure group(n=6)and treatment group(n=8). The acute failure model of pig was induced by D-galactosamine (dosage, 1.2g/kg). The treatment group was treated with NAC,and the acute failure group was administered with 0.9%NaCl. The blood ammonia, plasma endotoxin and inflammatory cell factor (TNF-α, IL-1β) were tested. Results: Compared with acute failure group, the level of blood ammonia and inflammatory cell factor were significantly decreased at 24 and 48 hours, plasma endotoxin was significantly fall at 12 hours in the treated group: blood ammonia (24 hours:112.5±21.7 vs 95.4±25.9, P<0.05; 48 hours: 257.5±50.7 vs 213.6±70.5, P<0.05), plasma endotoxin(12hours: 0.165±0.05 vs 0.13±0.01, P<0.05), TNF-α (24 hours: 24.0±5.3 vs 19.3±4.2, P<0.05; 48 hours: 29.2±8.7 vs 26.1±3.8, P<0.05), IL-1β(24 hours: 18.5±3.6 vs 15.2±3.4, P<0.05;48hours:24.3±6.3 vs 20.1±5.2,P<0.05). Conclusion: NAC can effectively protect hepatocyte function from injures, ameliorate ammonemia and endotoximia and inhibit Inflammatory reaction.
