104例慢性呼吸道疾病并发下呼吸道感染的病原菌调查

目的了解下呼吸道感染的病原菌及耐药情况,协助经验性抗感染治疗,避免药物滥用,从而防止细菌耐药。方法 统计2007 年9月~2008 年8月在兰州大学第一医院呼吸病房住院的104 例下呼吸道感染患者的临床资料、痰培养及药敏结果,患者均具有慢性呼吸道基础疾病。结果104例患者共行236次细菌培养,结果共分离出130株致病菌,其中革兰氏阴性杆菌83株(63.8%);革兰氏阳性菌9株( 7% ) ,真菌38株(29.2% )。COPD 患者感染的49株病原菌中53.1 %为革兰阴性杆菌感染,铜绿假单胞菌为最主要病原菌。支扩患者下呼吸道感染时革兰阴性杆菌占100 % ,铜绿假单胞菌是最常见的病原菌占58.3 %。肺结核患者合并感染时革兰阴性杆菌占77.8 % ,流感嗜血杆菌是最常见的病原菌占33.3 % 。结论患有慢性呼吸道疾病患者并发下呼吸道感染时的病原菌以革兰阴性杆菌为主,根据基础疾病、用药史和住院时间长短而变化,对病原菌的正确判断有助于临床合理治疗。

Objective To explore the bacteria distribution and drug resistance of pathogens among lower respiratory tract infection (LRTI) inpatients with underlying chronic respiratory tract diseases, to assist empirical anti-infective therapy and avoid drug abuse, in order to avoid bacterial resistance. Methods The clinical data ,sputum culture and drug susceptibility results of 104 LRTI patients with various underlying chronic respiratory tract diseases who were hospitalized Respiratory ward of the First Hospital of Lanzhou University during September 2007 ~ August 2008 were analyzed.. Results 104 cases of patients with a total of 236 bacterial cultures, isolated 130 pathogens, including Gram-negative bacilli 83 (63.8%); gram-positive bacteria 9 (7%), fungi 38 (29.2% ). Infection in COPD patients with 49 pathogens of 53.1% was Gram-negative bacilli infections, Pseudomonas aeruginosa as the most important pathogens; patients with bronchiectasis exacerbated due to bacterial infection ,100% were caused by gram negative bacteria with Pseudomonas aeruginosa (58.3%),as the most frequent pathogens; Patients with pulmonary tuberculosis co-infection, Gram-negative bacilli accounted for 77.8 percent, Haemophilus influenzae are the most common pathogens, accounting for 33.3%. Conclusions Gram negative bacteria were the main pathogens of LRTI in hospitalized patients with underlying chronic respiratory tract diseases. The pathogens were influenced by basic disease、medication and Length of hospital stay, Correctly judge pathogens will help to determine the correct and effective anti-infective treatment.
