背景 甲状腺功能亢进症(简称甲亢)与精神疾病发生相关,目前神经病理生理机制不明。 目的 本研究通过静息态功能磁共振成像(rs-fMRI)分析甲亢患者脑自发活动异常模式,并探讨甲亢患者脑损害的神经影像学特征。方法 纳入甲亢患者25例作为甲亢组及性别、年龄、受教育程度与甲亢组无显著差异的健康志愿者28例作为对照组。所有被试者均接受rs-fMRI检查,通过测量全脑比率低频振幅(fractional amplitude of low-frequency,fALFF)反映局部脑自发活动。使用SCL-90评估被试者的心理健康状况。应用相关方法分析mfALFF值与临床变量及心理症状之间的关系。结果 与对照组相比,甲亢组右侧小脑蚓部7区(MNI:3,-75,-27;t=4.3807)、左侧丘脑(MNI:-6,-6,0;t=3.5448)、右侧丘脑(MNI:6,-3,3;t=4.3559)的mfALFF值升高,左侧小脑后叶(MNI:-39,-81,-27;t=-3.4015)、左侧眶部额中回(MNI:-9,57,0;t=-5.2534)、左侧额下回(MNI:-51,15,18;t=-3.7407)的mfALFF值降低;相关分析发现,右侧小脑蚓部7区的mfALFF值与焦虑(P=0.037,r=-0.419)、敌对(P=0.046,r=-0.402)因子评分呈负相关。结论 右侧小脑蚓部脑自发活动异常可能是甲亢患者脑损害的重要影像学标记。

Background Hyperthyroidism is associated with the development of mental diseases, and the neuropathophysiological mechanism is unknown.? Objective In this study, resting state functional magnetic resonance imaging (rs-FMRI) was used to analyze the abnormal patterns of spontaneous brain activity in patients with hyperthyroidism, and to explore the neuroimaging characteristics of brain damage in patients with hyperthyroidism. ?Methods 25 patients with hyperthyroidism were enrolled as the hyperthyroidism group and 28 healthy volunteers with no significant difference in gender, age and education level were enrolled as the control group. All subjects underwent RS-FMRI, which measured fractional low-frequency amplitude of the whole brain ratio (fALFF) to reflect local spontaneous brain activity. The mental health of the subjects was assessed using SCL-90. Correlation method was used to analyze the relationship between mfALFF value and clinical variables and psychological symptoms. ?Results Compared with the control group, the hyperthyroidism group right cerebellar vermis region 7 (MNI: 3,-75,-27;t=4.3807), left thalamus (MNI: -6,-6,0;t=3.5448), right thalamus (MNI: 6,-3,3;t=4.3559), the left posterior cerebellar lobe (MNI: -39,-81,-27;t=-3.4015), left orbital middle frontal gyrus (MNI: -9,57,0;t=-5.2534) and left inferior frontal gyrus (MNI: -51,15,18;t=-3.7407) reduced the mfALFF value; Correlation analysis showed that mfALFF value of right cerebellar vermis region 7 was negatively correlated with anxiety (P=0.037,r=-0.419) and hostility (P=0.046,r=-0.402) factor scores. ?Conclusion Abnormal spontaneous activity in the vermis of the right cerebellum may be an important imaging marker of brain damage in patients with hyperthyroidism.
