目的:构建社区卫生服务风险评价指标体系,确定各个风险因素的风险等级,找出优先控制领域,为开展社区卫生服务风险管理提供理论依据。方法:通过文献综述、工作流程分析、现场问卷调查及专家访谈,筛选出社区卫生服务中存在的风险因素,利用REASON模型对风险因素进行分类,通过德尔菲专家咨询确定三级指标体系,再运用风险评价矩阵和Fisher判别分析确定各风险因素的风险等级。结果:构建的风险因素评价指标体系包括一级指标5项,二级指标13项,三级指标62项。三级指标中共包括一级风险因素6个,二级风险因素17个,三级风险因素30个,四级风险因素9个。结论:组织管理对于社区卫生服务的安全最为重要,其次是医务人员因素。所以,组织管理和医务人员素质是风险管理的优先领域。

Objective:To establish the risk evaluation system of community health service, determine the risk degree , establish the risk evaluation system of community health service , identify priority control areas and provide the basis for risk managemnt in commnity health service.?Methods:1. risk factors have been selected by on-site survey, work flow analysis, root-cause analysis, expert advice, expert interviews and other methods in 28 community health service agencies. 2. Reason model has been used to classify and summary the risk factors. 3. Delphi has been used to construct the risk evaluation system and RAC and Fisher discrimination analysis method to determine risk factors degree.Results:Risk factors evaluation system has been constructed, which included 5 first degree indicators, 13 second degree indicators and 62 third degree indicators. Fisher discrimination analysis to determine the risk factors rating, resulting 6 first degree risk factors, 17 second degree risk factors, 30 third degree risk factors and 9 fourth degree risk factors. Conclusion: The medical staff organization and management factors are most important for community health service safety and the two areas need to be focused on.
