雄激素受体(AR)在乳腺癌中的表达

摘要 目的:研究雄激素受体在乳腺癌组织中的表达及其与雌激素受体(ER) 、孕激素受体(PR) 、人体表皮生长因子受体2 (Her-2)及Ki67的相关性。方法:采用免疫组织化学的方法,检测雄激素受体在1069例乳腺癌组织中的表达,并对照分析雄激素受体与雌激素受体(ER) 、孕激素受体(PR) 、人体表皮生长因子受体2 (Her-2) 、Ki67、及其临床特征(患者年龄、肿瘤大小,肿瘤分级,分期淋巴结转移情况)等的相关性。结果:AR的阳性表达率明显高于ER、PR的阳性表达率。并且,肿瘤的分级、分期不同,AR的阳性表达率也会有所不同。在乳腺癌中,AR的阳性表达与ER、PR呈正相关,与肿瘤的大小、增殖指数呈负相关。当AR阳性而ER表达阴性时,AR与Her-2高度相关,与肿瘤的大小、Ki67呈负相关。当AR阳性而PR表达阴性时,AR与Ki67呈负相关。在Her-2表达阴性组中,AR的阳性表达率与Ki67呈高度相关。结论:AR在乳腺癌组织中呈高表达,与肿瘤大小、增殖指数呈负相关,与肿瘤的高分化显著相关,因此,AR可做为良好的预后指标,并成为目前激素治疗的新靶点。此外,在ER表达阴性的乳腺癌组织中,AR与Her-2的表达呈正相关。AR在乳腺癌组织中的潜在预测价值等很多方面都值得我们进一步的研究。
Instruction: Androgen receptor (AR) has been implicated to be important in breast carcinoma. The objective of this study is to investigate its relationship with estrogen receptor (ER), progesterone receptor (PR), HER2,and Ki67 in primary breast cancer in Chinese patients from a single institution. Methods: We evaluated the expression of AR, ER, PR, HER-2 and Ki67 by immunohistochemistry (IHC) in 1069 consecutive cases of breast cancer, along with their clinical features including patient age, tumor size, tumor type, tumor grade and lymph node status; and then analyzed the relationship between AR expression and these clinicopathologic factors. Results: Expression of AR higher than that of ER and PR, and was highest in carcinoma in situ. Its expression also differed among various types of breast carcinoma with highest among invasive lobular carcinoma and mucinous carcinoma and lowest with medullary carcinoma. AR expression was significantly correlated with small tumor size, low proliferation index and expression of ER and PR. In ER negative tumors, AR was strongly associated with HER2 over-expression as well as expression of PR, low Ki-67, and small tumor. In PR negative tumors, AR was strongly associated with ER and low Ki-67, but not with HER2 over-expression and tumor size. In HER2 negative tumors, AR was strongly associated with ER, PR, low Ki-67, but not with tumor size. Conclusion: AR expression in Chinese patients is associated with small tumor size, favorable differentiation and low proliferation index; and there are significantly different expression patterns for AR in ER, PR, HER2 positive and negative tumors, indicating their close relationship in breast carcinogenesis. Further studied are needed for investigate the inter-relationship among these factors in breast carcinomas.
