慢性乙型肝炎肝脏磁共振31磷波谱与Child-Pugh分级的对照研究

目的 磁共振31磷波谱（31P Magnetic Resonance Spectroscopy ，31P-MRS）是一种可用作研究活体肝脏的细胞水平的能量代谢和生化指标变化的非侵入检查方法。本研究探讨不同Child-Pugh分级的慢性乙型病毒性肝炎患者中，31P-MRS指标是否存在差异，能否反映肝炎的严重程度。方法 收集慢性乙型病毒性肝炎患者30例，行肝脏31P-MRS扫描，计算肝脏各种磷化合物含量，包括PME、PDE、PCr、Pi、γ-ATP、β-ATP和α-ATP。30例肝炎患者按Child-Pugh分级，比较不同级别患者的肝脏31P-MRS指标。结果 本组肝炎患者Child-Pugh A级有26例，B级有4例。PME%和PME/PDE在各级之间差异有统计学意义，A级PME%为0.170±0.039，PME/PDE为0.96±0.23；B级PME%为0.235±0.026，PME/PDE为1.72±0.49。其余指标各级之间差异无统计学意义。结论 随Child-Pugh级别升高，31P-MRS测得的肝脏PME%和PME/PDE升高。表明31P-MRS可反映慢性乙型病毒性肝炎的严重程度。
Objects 31P-MRS is an noninvasive method providing metabolic and biochemical information of liver at a cellular level. We sought to investigate the correlations between 31P-MRS and the Child-Pugh grading of the chronic type-B viral hepatitis, and to assess the evaluation of 31P-MRS in reflecting the severity of type-B viral hepatitis. Methods 30 patients of type-B viral hepatitis were underwent 31P-MRS of the liver. The following data were acquired from 31P-MRS: The concentration of the phosphorus compounds of the liver, including PME, PDE, PCr, Pi, γ-ATP, β-ATP and α-ATP. The patients were grouped by CHILD-Pugh grades, the 31P-MRS data were compared among different grades. Results Of the 30 patients, 26 were CHILD-Pugh grade A, 4 were grade B.The differences of PME% and PME/PDE between grade A and grade B had statistical significance. In CHILD-Pugh A, PME% was 0.170±0.039，PME/PDE was 0.96±0.23；in CHILD-Pugh B, PME% was0.235±0.026，PME/PDE was1.72±0.49。The difference of other data showed no statistical significance between the two grades. Conclusion PME% and PME/PDE increased along with the increasing of the CHILD-Pugh grades in the type-B viral hepatitis patients. So that 31P-MRS can evaluate the severity of type-B viral hepatitis.
