目的 探讨胸苷激酶1(TK1)、增殖细胞核抗原(Ki-67)在三阴性乳腺癌(TNBC)中的表达及其与预后的关系。方法 选取2008年6月至2014年12月期间连续获保乳根治术或改良根治术的乳腺癌患者的肿瘤组织切片120例,其中三阴性乳腺癌(TNBC)60例,非三阴性乳腺癌(NTNBC) 60例。应用免疫组化法检测乳腺癌组织中TK1、Ki-67的表达,并分析二者在TNBC中的表达与患者年龄、肿瘤大小、绝经状态、组织学分级、淋巴结转移以及患者预后的关系。结果 TK1在TNBC中的表达为83.33 % ,在NTNBC中的表达为51.67 %,二者有统计学差异(χ2 = 13.713,P = 0.000)。Ki-67 在 TNBC 中的表达为68.33 %,在NTNBC中的表达为31.67 %,二者有统计学差异(χ2 = 16.133,P = 0.000)。TNBC患者TK1的表达与组织学分级有相关性,差异有统计学意义(P = 0.013),与发病年龄、绝经状态、肿瘤大小、淋巴结转移无关。TNBC病人Ki-67的表达与组织学分级(P = 0.013)、淋巴结转移(P = 0.000)相关,差异均有统计学意义,与绝经状态、发病年龄、肿瘤大小无关。TNBC患者TK1阳性表达与Ki-67阳性表达呈正相关(χ2 = 5.143;r = 0.369,P = 0.023)。Kaplan-Meier生存分析显示TK1和Ki-67阳性表达患者的预后均比阴性表达患者差,有统计学差异(P = 0.041、0.033) 结论 TK1、Ki-67在TNBC中的表达与NTNBC相比差异显著,其在TNBC中均高表达,且表达水平均与组织学分级有相关性,有助于判断预后,成为新的预后指标,为预测TNBC的临床及生物学特征提供依据及研究方向。

Objective To analyze the expressions of Thymidine Kinase 1(TK1) and nuclear-associated antigen Ki-67(Ki-67) in triple negative breast cancer(TNBC) and their prognosis. Methods There were 120 cases collected randomly,each group was 60 cases.The expression of TK1 and Ki67 in different breast tissue were detected by IHC technique. The relations of the two expressions with patient's age, tumor size, menopausal state, histological staging, lymphatic metastasis and the prognosis of patients were analyzed. Results: The positive rate of TK1 expression in TNBC was 83.33%,and that in non-TNBC was 51.67%, the difference being significant(χ2 = 13.713, P = 0.000). The positive rate of Ki-67 expression in TNBC was 68.33%, and that in non-TNBC was 31.67%, the difference being significant(χ2 = 16.133, P = 0.000). In TNBC, the expression of TK1 was related with histological staging (P = 0.013), and that of Ki-67 was related with histological staging(P = 0.013) and lymphatic metastasis(P = 0.000). The expression of TK1 was positively related with that of Ki-67 (χ2 = 5.143; r = 0.369, P = 0.023). The results of survival analysis showed that positive expression of both TK1 and Ki-67 represent worse prognosis than that not. Conclusion: The expressions of TK1 and Ki-67 in TNBC was different from that in non-TNBC, are expected to become prognostic indicators, and thereby, provide a direction of characteristics and endocrine therapy.
