目的:我们发现肿瘤患者外周血中存在着一种特殊细胞,其既表达B淋巴细胞的表面标志物CD19,又表达T淋巴细胞的表面标志物CD3,我们称之为B细胞样T细胞(CD19+CD3+,以下简称B-T细胞),并与健康志愿者比较分析,分析其在肿瘤患者中的表达规律和可能的价值。方法:回顾性分析2013年5月至2016年2月于天津中医药大学第一附属医院肿瘤科住院的恶性肿瘤患者267例,同时选择同期健康志愿者62例作为对照组。应用流式细胞术检测外周血中CD19+CD3+细胞的水平,比较两者间的差异。结果:健康志愿者外周血中B-T细胞百分率为0.12%,肿瘤患者外周血中B-T细胞百分率为0.45%,肿瘤患者B-T细胞百分率显著高于健康志愿者,差异具有统计学意义(P<0.01),且肿瘤患者B-T细胞百分率的P25远高于健康志愿者的P75,具有显著差异。肿瘤患者外周血中B-T细胞百分率与淋巴细胞亚群无明显相关性(P>0.05)。肿瘤组中不同瘤种B-T细胞百分率不同,食管癌、非霍奇金淋巴瘤、肝癌较高,卵巢癌和肾癌较低,且最高的食管癌与最低的肾癌之间比较有显著性差异(P<0.05)。胃癌、卵巢癌、胰腺癌中B-T细胞分布较集中,而肝癌、食管癌、乳腺癌B-T细胞分布较分散。肿瘤组中不同年龄、不同性别、不同临床分期、有无转移、有无淋巴结转移、有无脾大的患者B-T细胞百分率无显著差异(P>0.05)。结论:健康志愿者与肿瘤患者外周血中均存在B-T细胞,且肿瘤患者B-T细胞百分率显著高于健康志愿者,有可能成为肿瘤免疫损伤的标志物。

Objective: We found that there is a special cell in the peripheral blood of cancer patients, which express both the surface marker CD19 of B lymphocytes and the surface marker CD3 of T lymphocytes, which we call B cell-like T cells (CD19+CD3+, hereinafter referred to as B-T cells), and compared with healthy volunteers, and analyzed its expression and possible value in cancer patients. Methods:A total of 267 patients with malignant tumor were enrolled in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from May 2013 to February 2016, and 62 healthy volunteers were recruited as the control group.The levels of CD19+CD3+ cells in peripheral blood were measured by flow cytometry, and the differences between the two groups were compared. Results: The percentage of B-T cells in peripheral blood of healthy volunteers was 0.12%. The percentage of B-T cells in peripheral blood of cancer patients was 0.45%,and the percentage of B-T cells in cancer patients was significantly higher than that in healthy volunteers, the difference was statistically significant (P <0.01).And P25 in the percentage of B-T cells in cancer patients was significantly higher than P75 in the percentage of B-T cells in healthy volunteers,with significant differences.There was no significant correlation between the percentage of B-T cells and lymphocyte subsets in peripheral blood of cancer patients (P> 0.05).The percentages of B-T cells in different tumor species were different. The percentages of B-T cells were high in esophageal cancer, non-Hodgkin's lymphoma and liver cancer, and were low in ovarian cancer and renal carcinoma,and there was significant difference between the highest esophageal carcinoma and the lowest renal carcinoma (P <0.05).The distribution of B-T cells in gastric cancer, ovarian cancer and pancreatic cancer was more concentrated, but the distribution of B-T cells in liver cancer, esophageal cancer and breast cancer was more dispersed.For cancer patients,there was no significant difference in the percentage of B-T cells in different age groups, different sexes, different clinical stages, with or without metastasis, lymph node metastasis, and splenomegaly (P> 0.05). Conclusion:B-T cells were present in peripheral blood of healthy volunteers and cancer patients, and the percentage of B-T cells in cancer patients was significantly higher than that of healthy volunteers, which could be a marker of tumor immune injury.
