结核分枝杆菌持续感染致T细胞功能障碍的研究

目的 探究结核分枝杆菌长期感染持续激活机体免疫应答是否造成T细胞功能障碍,研究导致慢性重症结核病患者对结核分枝杆菌免疫力降低的机制。方法 抽取兰州市肺科医院肺结核患者35例,其中12名为痰菌阳性的空洞性肺结核患者。分离外周血淋巴细胞,通过酶联免疫斑点试验(ELISPOT) 和酶联免疫吸附试验(ELISA )分析T 淋巴细胞 IFN-γ 和 IL-2 分泌水平;用细胞增殖试验分析T细胞的增殖能力;用流式细胞术检测CD4+、CD8+ T细胞表面免疫抑制相关分子Tim-3的表达。 结果 和健康大学生相比,痰菌阳性的空洞性结核病患者T细胞分泌IL-2的能力显著降低,T细胞的增殖能力明显下降,细胞表面免疫抑制相关分子Tim-3的表达显著增高。与痰菌阳性的非空洞性结核病患者相比,痰菌阳性的空洞性结核病患者T细胞分泌IFN-γ能力显著降低。 结论 结核分枝杆菌的持续感染会造成患者T淋巴细胞的功能减弱,发生T淋巴细胞功能障碍。

Objective To explore whether the long-term infection sustained activation of the immune response leads to T-cell dysfunction or not and study the mechanism which chronic severe TB patients lower immunity against Mycobacterium tuberculosis. Methods We collected 35 cases tuberculosis patients from Lanzhou Pulmonary Hospital,including 12 sputum smear-positive cavitary TB patients. Analyze the level of T lymphocyte secreting IFN-γ and IL-2. Analyze the ability of T cell proliferation by cell proliferation assay. Detect the expression of CD4+, CD8+ T cell surface immunosuppression related molecules Tim-3 by flow cytometry. Results For sputum smear-positive cavitary TB patients, the abilities of T cells secreting IFN-γ and IL-2 were significantly reduced, the proliferation of T cells was significantly weakened, the expression of immunosuppression-related molecules Tim-3 was significantly increased comparing with the healthy donors. For sputum smear-positive cavitary TB patients, the abilities of T cells secreting IFN-γ were significantly reduced comparing with the sputum smear-positive non- cavitary TB patients. Conclusion With persistent Mycobacterium tuberculosis infection, the T lymphocytes function in patients was waning. What's more, the exhaustion of T lymphocyte function happened finally.
