补肾缩尿法对OAB豚鼠膀胱逼尿肌功能及其缝隙连接蛋白Cx43mRNA表达的影响

目的:研究肾气丸联用缩泉丸对膀胱过度活动症豚鼠模型膀胱逼尿肌功能和膀胱缝隙连接蛋白Cx43 mRNA表达的影响。方法:48只雌性豚鼠随机平均分成假手术组、模型组、托特罗定阳性组、缩泉丸联用肾气丸组和缩泉丸组。手术组在麻醉下用直径1mm 的硬膜外麻醉导管由尿道插入膀胱作支架后,沿尿道外口做"O"字形切口切开皮肤,游离尿道,用3/0 丝线结扎游离的尿道;假手术组仅做插管,切开皮肤, 游离尿道,但不结扎尿道。从造模第 2天,给药组开始灌胃给予缩泉丸、肾气丸、缩泉丸联用肾气丸连续 2周,正常对照组、模型组分别灌胃给予同体积蒸馏水,阳性对照组灌胃给予同体积托特罗定,给药周期同给药组。两周后取膀胱肌条称量,进行离体逼尿肌实验,依次加入α,β-me ATP、不同浓度carbachol、KCL激动剂,观察不同给药组肌条对于不同激动剂收缩作用的改变;并称取100mg膀胱组织,用荧光定量PCR法测定缝隙连接蛋白Cx43mRNA表达水平。结果:缩泉丸联用肾气丸能显著性降低由于KCL激动剂所引起的肌条收缩作用(P<0.05),并在Carbachol不同浓度影响下的收缩作用均有显著性降低(P<0.05);缩泉丸联用肾气丸干预后,膀胱缝隙连接蛋白Cx43的表达量呈极显著下降(P<0.01)。结论:补肾缩尿法可能通过降低膀胱缝隙连接蛋白Cx43的表达,调节膀胱逼尿肌功能从而起到改善膀胱过度活动症。
Objective: To explore effects of the method with tonifying kidney and reducing urination on bladder detrusor function and gap junction protein Cx43 mRNA expression of the overactive bladder (OAB) model of guinea pig.Method: forty-eight female guinea pigs randomly divided into control, model, tolterodine, shenqiwan, suoquanwan, shenqiwan combinating with suoquanwan groups, respectively eight. The obstruction was done in the experimental group by 3/ 0 silk thread tying around the epidural block catheterized urethra loosely from an "O" cut following the urethra after anesthesia, while the same operation except tying around the urethra was done in control group. Treatment of SQW, Shenqiwan, and shenqiwan combining with suoquanwan was carried out from the 2nd day after the operation. Normal control group, model group respectively were given the same volume with distilled water. The positive group were given by gavage with the same volume of tolterodine, and lasted for 2 weeks. After two weeks, the bladder was fetched into strips, Weighed and recorded of mechanical activity in vivo. To join the agonist of α, β- me ATP, different concentration of carbachol, KCL in turn, observe the change of different agonists contraction effect acting on the different dosage group's strips. 100 mg of the urinary bladder was used to determine the gap junction protein Cx43mRNA with fluorescent quantitative PCR.Result: shenqiwan combinating with suoquanwan can significantly reduce the contraction caused by KCL agonist (P<0.05), and the contraction function under the influence of different concentrations of carbachol were significantly lower than OAB models (P<0.05). Meantime, expression of the bladder gap junction protein Cx43 was significantly decreased by given shenqiwan combinating with suoquanwan (P<0.01). Conclusion: the method with tonifying kidney and reducing urination may reduce the expression of bladder gap junction protein Cx43 and adjust the bladder detrusor function to improve the overactive bladder.
