经皮椎间孔镜与微创经椎间孔腰椎融合术治疗单个腰椎间盘突出的围手术期观察

Objective: To compare the outcomes during the perioperative of percutaneous endoscopic lumbar discectomy(PELD) versus MIS-TLIF via Quadrant minimally invasive system in the treatment of single level lumbar disc herniation. Methods: Retrospect the Jan.2014 to may.2015, a total of 41 patients with single level lumbar disc herniation were divide into 2 groups. Twenty-two cases in group PELD underwent percutaneous endoscopic lumbar discectomy and nineteen cases in group MIS-TLIF underwent MIS-TLIF via Quadrant minimally invasive system. Operative time, blood loss, volume of drainage after operation and hospital stay were compared. Results: There were no statistical differences in gender, age, disease stage，clinical diagnosis and imaging examination. Compared with the control group, the observation group has shorter operation time [(123.18 + / - 40.046 min) vs (238.95 + 50.238) min t = 8.207, p = 0.000), less intraoperative blood loss [ml (20.81 + 6.49) vs (245.00 + 185.03) for ml, t = 5.278, p = 0.000), less postoperative fluid volume [ml (8.86 + 5.00) vs (115.42 + 62.65) for ml, t = 7.393, p = 0.000) and shorter hospitalization time [d (10.09 + 2.369) vs (16.26 + 4.771) d, t = 5.121, p = 0.000].Conclusion :PELD has advantages of shorter operation time, less intraoperative blood loss, less postoperative fluid volume and shorter hospitalization time in treating single-level lumbar disc herniation.
