叶酸、维生素B12、普罗布考与原发性高血压干预治疗的研究
Study on relationship among Folic acid ,VitaminB12,Probucol about the Intervention Treatment In Essential Hypertention
目的 探讨叶酸、VitB12和/或普罗布考在原发性高血压治疗前后血浆不对称二甲基精氨酸(ADMA)、同型半胱氨酸(Hcy)指标的内在联系,探索高血压的发病机制及其预防脑卒中等并发症的措施。方法 采用病例-对照研究方法,将原发性高血压患者120例随机分为三组。第一组常规降压、第二组在第一组治疗的基础上给予叶酸+VitB12,第三组常规降压同时给叶酸、VitB12与普罗布考。健康人30例作为对照组。观察治疗前基线、2周及12周ADMA、Hcy、NO和eNOS的变化。结果 原发性高血压组患者血浆ADMA、Hcy浓度均显著高于健康对照组两者的浓度 (P<0.01);第一组治疗前后血浆ADMA、Hcy浓度差异无显著性(P>0.05);第二组治疗12周ADMA、Hcy浓度比基线值、治疗2周明显减低(P<0.01),血清NO、eNOS浓度明显升高(P<0.01)。第三组治疗治疗2周时及12周ADMA、Hcy浓度均明显减低(P<0.01)。ADMA 与Hcy 呈正相关(P<0.05)。结论 给予高血压患者补充适量的叶酸、VitB12和/或普罗布考可能有助于通过阻断ADMA、Hcy对血管内皮细胞损伤的恶性循环,改善高血压患者的血管内皮细胞功能,减缓小动脉硬化的进展,预防心脑血管事件。
Objective To investigate the internal relationship among ADMA、Hcy after the supplementation of folic acid、vitamin B12 and probucol to explore mechanism of hypertension and preventive measures. Method A total of 120 patients with hypertension were randomly divided into three groups. Among the first group we gave only anti-hypertensive treatment, and the second group on the basis of treatment of the first group were given folic acid and VitB12. The third group on the basis of treatment of the second group were given probucol. Additionally,30 healthy people were selected as control group. Observe changes of ADMA、Hcy among the baseline、2 weeks and 12 weeks. Results Plasma ADMA、Hcy concentration in patients with essential hypertension were both significantly higher than the concentration of control group (P<0.01);In the first group concentrations of plasma ADMA、Hcy had not significantly differences(P>0.05);In the second group plasma ADMA、Hcy concentrations after treatment of 12 weeks had more significantly differences than the baseline and treatment of 2 weeks (P<0.01);In the third group plasma ADMA、Hcy concentrations after treatment of 2 weeks had more significantly differences than the baseline (P<0.01), after treatment of 12 weeks they had more significantly differences than the baseline and treatment of 2 weeks (P<0.01). Plasma levels of ADMA were positively correlated with Hcy (P<0.05). Conclusions The supplementation of folic acid、VitB12 and/or probucol may help improve endothelial function and inhibit the vicious circle by the reduction of ADMA and Hcy,slowing the progress of atherosclerosis and preventing cardiovascular events in patients with hypertension.
