早期坏死性巩膜炎的UBM观察
Ultrabiomicroscopy in the early detection of necrotic sceleritis
目的:了解超声生物显微镜(Ultrasoud biomicroscope, UBM)在坏死性巩膜炎早期诊断和随访中的作用。方法:6例(6眼)临床确诊为坏死性巩膜炎的患者在药物或手术治疗前后接受了UBM检查,以观察病变发展和消退过程中病变区巩膜及其周围组织的动态变化。随访时间为(6~18)月。 结果:坏死性巩膜炎活动期,UBM表现为病灶中央低回声或无回声区形成,周围巩膜实质增厚,回声增强,病灶内侧睫状体组织增厚,回声增强,部分患者伴有局限性睫状体脱离。在病情缓解后1月时,UBM表现为病灶中央低回声或无回声区缩小,周围巩膜实质回声恢复正常,病灶内侧脱离的睫状体组织复位。最后一次随访时,UBM仅表现为病变区巩膜明显变薄。结论:UBM可以动态观察坏死性巩膜炎的病情变化,并可作为早期诊断的辅助检查手段。
Objective: To evaluate the efficacy of ultrasoud biomicroscopy (UBM) in the early detection of necrotic sceleritis.Methods: UBM was used in 6 cases who were diagnosed as early necrotic sceleritis with a follow-up time of 6~18 months. The dynamic changes of scleral focus and surrouding tissues were detected by UBM before and after the medical or surgical therapy of necrotic sceleritis. Resuts: The UBM findings in the inflammation stage of necrotic sceleritis included the low-echo in central focus which was surrouded by thickenned and high-echo tissues and ciliary detachment in some cases. When the inflammation resolved, the scleral became thinner. Conclusion: UBM is useful in early detecting of necrotic sceleritis .
