目的 回顾性分析不同支持模式下成人体外膜氧合(ECMO)支持期间血小板计数变化规律。方法 回顾性纳入2019年1月至2021年10月在高州市人民医院使用ECMO支持的40例成人患者,根据应用场景分为心脏术后ECMO支持组(手术组,n=17)和非心脏手术ECMO支持组(非手术组,n=23),采用广义估计方程式的线性回归模型分析各独立变量与血小板跨时间变化的回归模型。结果 在两组患者中血小板计数变化呈曲线、双时间模式,在ECMO启动后第2~3 d开始下降,至3~4 d达到谷底,随后血小板曲线回升并呈现不同的预后转归曲线变化,即成活组较死亡组有着相对高的血小板数值和较稳定的数值回升曲线。所有患者平均输血小板量为(1.60±2.00)U,而首次输入后有近六成(59.09%)患者的血小板达到有效升高。结论 ECMO支持下的血小板变化呈曲线、双时间模式,通常于第3~4 d达到最低值,患者预后转归与血小板的回升存在相关性。

Objective Retrospective analysis of platelet count changes in adults during different ECMO support. Methods 40 adult patients who received ECMO support in Gaozhou People's Hospital from January 2019 to October 2021 were retrospectively enrolled in the study. According to application scenarios, they were divided into ECMO support group after cardiac surgery (surgery group, n=17) and ECMO support group without cardiac surgery (non-surgery group, n=23). Results The changes of platelet count in the two groups showed a curve and dual-time pattern, which began to decline 2-3 days after the initiation of ECMO and reached the bottom 3-4 days later, and then the platelet curve rose and showed different prognostic curve changes, that is, the survival group had a relatively higher platelet value and a more stable value rise curve than the death group. The mean platelet transfusion volume of all patients was (1.602.00) units, and nearly 60% (59.09%) patients achieved effective platelet elevation after the first infusion. Conclusion The platelet changes supported by ECMO showed a curve and dual time pattern, and usually reached the lowest value on the 3rd to 4th day. The prognosis of patients was correlated with platelet rebound.
