α-淀粉酶抑制剂(白芸豆提取物)对2型糖尿病患者生理代谢及肠道菌群的影响

本研究在2型糖尿病患者的膳食营养干预中引入白芸豆淀粉酶抑制剂(α-AI),研究其对2型糖尿病生理代谢指标及肠道菌群的影响,为糖尿病的营养干预提供更简便、有效的方案。对2型糖尿病患者的膳食营养干预,引入白芸豆α-AI提取物,并辅以糖尿病膳食宣教、血糖监测等综合干预手段,研究其对2型糖尿病生理代谢指标的影响,并通过研究其肠道菌群的变化,揭示白芸豆α-AI提取物在体内改善糖代谢的机理。结果显示,患者经白芸豆α-AI提取物干预后,血糖控制良好,糖化血红蛋白(HbA1c)、空腹血糖和餐后血糖显著降低。低密度脂蛋白(LDL-c)下降幅度显著大于对照组,高密度脂蛋白(HDL-c)升高幅度显著大于对照组。服用白芸豆α-AI提取物对患者糖尿病并发症亦有改善作用。白芸豆提取物通过减少消化系统对淀粉的吸收,使更多淀粉到达大肠,在干预2个月时调节肠道菌群结构的能力最强,而到4个月时调节作用减弱。干预前后,菌群的α-多样性均未发生显著变化,菌群β-多样性与对照组相比均存在着显著差异(干预2个月时,p<0.05;干预4个月时,p<0.01)。白芸豆提取物能在干预2个月时显著富集Bifidobacterium、Faecalibacterium等富含有益菌的菌属,降低Escherichia/Shigella这一般被认为含有潜在机会致病菌的菌属的丰度,而到4个月时,仅显著富集Bifidobacterium菌属。因此,白芸豆α-AI提取物对糖尿病人血糖的调节,除了其直接的降低淀粉的吸收外,还有可能通过改变肠道菌群的结构起作用。
Based on diabetes diet education, blood glucose monitoring and other comprehensive intervention, the α-AI extract of white kidney bean was added to dietary of type 2 diabetes patients. Its effect on the physiological and metabolic indicators of type 2 diabetes were studied, and the mechanism of white kidney bean α-ai extract in vivo by studying the changes of its intestinal flora have been reaveled.The results showed that the blood glucose was well controlled after the intervention of white kidney bean α-AI extract, and the glycosylated hemoglobin (HbA1c), fasting blood glucose and postprandial blood glucose were significantly decreased. The decrease of low-density lipoprotein (LDL-c) was significantly larger than that of the control group, on the contrary, the increase of high-density lipoprotein (HDL-c) was significantly larger than that of the control group. The diabetic complications also were improved after eating white kidney bean α-AI extract. After eating white kidney bean extract, the absorption of starch in the digestive system was reduced, so that more starch to the large intestine. Compared to 4 months intervention, the change of structure of intestinal flora was more significant foe 2 months intervention. The α- diversity of flora did not change significantly before and after intervention, and the diversity β- flora was significantly different compared with the control (2 months of intervention ,4 months of p<0.05; intervention, p<0.01). After adding white kidney bean extract 2 months, Bifidobacterium, Faecalibacterium and other beneficial bacteria wrere significantly enriched, and the abundance of Escherichia / Shigella, which is generally considered to contain potential opportunistic bacteria, were reduced. However, only Bifidobacterium was significantly enriched at 4 months. .
