基于智慧健康处方的社区全程化健康管理模式研究
Research on Community Whole-Course Health Management Model Based on Intelligent Health Preion
针对健康不同阶段、不同的健康问题和个性化健康需求，融合八个维度的健康处方，研究建立覆盖全生命周期、匹配200多个病种的新型智慧健康处方，完成健康处方系统的研发，并选取北京市东城区8个社区卫生服务机构进行试点应用，为实现全程化健康管理、创新型健康管理模式提供依据。

Aiming at different stages of health, different health problems and personalized health needs, eight dimensions of health preions were integrated to establish a new intelligent health preion covering the whole life cycle and matching more than 200 diseases, complete the research and development of health preion system, and selects eight community health service institutions in Dongcheng District, Beijing to carry out pilot applications, so as to provide a basis for realizing whole-process health management and innovative health management mode.
