中国社区老年人潜在不适当用药率及相关因素的Meta分析

BackgroundWith the aggravation of population aging, the comorbidity and polypharmacy are increasing in older adults, which may increase the risk of potentially inappropriate medication. ObjectiveTo systematically assess the prevalence and associated factors of potentially inappropriate medication in community-dwelling older adults in China in order to provide reference for developing intervention strategies. MethodsWe searched databases of CNKI, Wanfang, VIP, CBM, PubMed, Embase, and Web of Science from inception to June 2022 for relevant studies on potentially inappropriate medication in community-dwelling older adults in China. Two independent reviewers selected studies, extracted data and assessed the quality of studies. Meta-analysis was conducted using Stata 12.0. ResultsA total of 24 studies were included. Meta-analysis showed that the pooled prevalence of potentially inappropriate medication was 34.8% [95%CI31.3%38.3%] in community-dwelling older adults in China. Pooled estimates showed that age75 years or above[OR=1.261, 95%CI1.074, 1.481], comorbidity5[OR=3.287, 95%CI1.405, 7.691], number of drugs5[OR=1.800, 95%CI1.305, 2.481] and number of drugs10[OR=4.380, 95%CI2.612, 7.347] were associated factors of potentially inappropriate medication. ConclusionPotentially inappropriate medication is common in community-dwelling older adults in China. Prevention and management of potentially inappropriate medication should be paid attention in this population.
