目的: 比较自然分娩与剖宫产产妇产后2~5天自评泌乳量、产后6h~24h血清中催乳素(PRL)以及初乳中胰岛素样生长因子(IGF-1)、表皮生长因子(EGF)浓度的差异。方法: 在医院产科对招募的志愿者收集产后6h~24h的血清以及产后2~5天的初乳。采化学发光免疫分析法测定自然分娩组与剖宫产组PRL的含量,采用生物素-亲和素酶联免疫吸附试验法测定两组初乳中IGF-1和EGF的浓度。结果: 自然分娩组产后血清PRL浓度为361.67±80.74ng/ml,剖宫产组为221.94±90.82ng/ml,两组之间差异有统计学意义(P <0.05)。产后2~3天,剖宫产组初乳中IGF-1浓度显著高于自然分娩组(P <0.05),产后4~5天,两组IGF-1含量差异无统计学意义(P>0.05)。产后2~5天,剖宫产组EGF浓度要略高于自然分娩组,但两组差异无统计学意义(P>0.05)。结论: 剖宫产组产妇早期初乳中较高的IGF-1以及血清中较低的PRL水平与泌乳延迟和泌乳量相关,建议剖宫产后仍应坚持早接触、早吸吮,提倡早期母乳喂养。

Objective To compare the difference in serum prolactin levels and self-estimated milk yield and the concentrations of growth factors of human colostrum from 2d to 5d after delivery between spontaneous labor and cesarean section. Methods The serum prolactin levels were measured during 6-24 hours after parturition in the spontaneous labor and cesarean setion using chemiluminescent immunoassay, and the levels of IGF-1, EGF are tested at the same time using the technique of ABC- ELISA. Results There was a significantly lower postpartum prolactin (PRL) level in the caesarean section group (221.94 ± 90.82ng/ml) compared with spontaneous labor group (361.67±80.74ng/ml) (P<0.05). The levels of IGF-1 of the colostrum in the cesarean section group were higher significantly than those of the spontaneous group from 2d to 3d after delivery (P<0. 05) and the self-estimated milk yeld in the cesarean section group were lower significantly than spontaneous group from 2d to 3d after delivery (P<0. 05). From 4d to 5d after parturition, there was no significant difference in IGF-1 and self-estimated milk yeild between the two groups (P>0.05). Meanwhile, there was no significant difference in EGF between the two groups from 2d to5d after parturition (P>0.05). Conclusions During 6-24 h after parturition, the serum prolactin levels of normal labor group are higher than that of the cesarean section group. The growth factors in human early colosrum of cesarean section group are higher than that of the spontaneous labor group. Caesarean section is associated with significantly lower postpartum PRL and the higher IGF-1, which is related to the longer breastfeeding initiation and lower milk yield. Necessary measures including promoting the secretion of postpartum PRL such as early contact and early sucking should be taken to improve the breastfeeding.
