散发性乳腺癌组织中乳腺癌易感基因表达相关因素分析

本文检测121例散发性乳腺癌和20例乳腺良性病变组织石蜡标本的BRCA1蛋白、ER、HER-2的表达,并结合患者的年龄、原发肿瘤的病理类型和腋淋巴结转移状况等病理学指标,分析影响BRCA1蛋白表达的因素及与临床病理特征之间的相关性。研究表明:乳腺良性病变组织中BRCA1、ER蛋白均在细胞核阳性表达;散发性乳腺癌组织中,BRCA1、ER蛋白表达明显降低,BRCA1、ER蛋白表达在良、恶性乳腺病变组织之间均有显著性差异(p﹤0.05)。HER-2阳性表达的乳腺癌组织中,BRCA1阳性表达率降低,BRCA1蛋白表达与HER-2过表达呈负相关。BRCA1蛋白表达与年龄呈负相关,低年龄组BRCA1蛋白表达阳性率高于高年龄组(p﹤0.05);BRCA1蛋白表达与腋淋巴结转移呈负相关,有腋淋巴结转移的BRCA1表达降低的更显著(p﹤0.05);BRCA1蛋白表达与乳腺癌的病理类型未见明显相关性。研究结果提示,检测BRCA1蛋白水平结合ER、HER-2的表达和临床资料,对乳腺癌的诊断和判断预后有指导意义。
In this paper we selected 121 sporadic breast carcinoma and 20 benign breast diseases, the expression level of BRCA1, ER and HER-2 protein were detected by immunohistochemistry (IHC), analysis the factors affect the expression of BRCA1, and the correlation between expression of BRCA1 and the clinical parameters. In the benign breast tissues, the positive expression of BRCA1 and ER protein in the nuclei. While in the breast carcinoma tissues, there are low or loss expression of BRCA1 and ER proteins, the differences was significant (p﹤0.05). There are negative correlation between the expression of BRCA1 and HER-2. The expression of BRCA1 in breast carcinoma was correlated with the age of patient. There are no significant differences of expression of BRCA1 between the each pathology types. The expression of BRCA1 protein negatively correlates to the metastasis of axillary’s lymph nodes. So binding with the expression of ER and HER-2, connecting with the clinic parameters, the detection of BRCA1 expression benefits the assessment of the prognosis of breast cancer.
