Objectiv: To explore the major effect factors of serum potassium in myeloproliferative neoplasms (MPN)and provide the best method to identify pseudohyperkalemia for clinical diagnosis. Methods: Blood routine and biochemical results of 268 MPN cases and 60 cases of healthy people were statistically analysed, and plasma potassium, blood gas analysis were measurd in 20 patients with elevated serum potassium. Results: in MPN, a highly significant positively correlation (P=0.000, 0.002, 0.000) were observed the platelet counts and the serum potassium level; There were no obvious correlation (P=0.647, 0.093, 0.039) between hematocrit and serum potassium; There were significantly negative correlation(P=0.000)between white blood cell counts and serum potassium in chronic myelogenous leukemia(CML) group; there were negatively correlation(P=0.001) between blood glucose and the serum potassium level level in essential thrombocytosis (ET) group. Blood gas analysis results showed that the whole blood potassium correction values were in the normal range (2.77-5.16mmol/l) in 20 patients with elevated serum potassium, Therefore, the blood gas analysis of whole blood potassium could reflect the real blood potassium level in vivo. Conclusion: In MPN, the increase platelet counts in blood components was the main reason for Pseudohyperkalemia; At the same time, the blood glucose can be counted to identify Pseudohyperkalemia; The blood gas analysis of whole blood potassium could reflect the real blood potassium level in vivo. blood potassium level in vivo.
